- Observation (CORRECTED DURING INSPECTION): 1. Found meat slicer in back room to have grease build up and mice droppings on it. Currently not being used.
- 2. Pizza pans in the back have oil on them.
- 3. Spray bottle had in excess of 200 ppm chlorine.
- Corrective Action(s): 1. wash and sanitize meat slicer before putting back in the back room. If no longer needed as meat comes pre-sliced then remove from facility.
- 2. Ensure all oil is removed from pans before putting them in the back storage area.

- Observation: Observed bait stations throughout premise including the kitchen, dishwashing area and food storage areas. Pest control report states that bait stations were placed in the interior of the building. Product specification for the bait placed was available on site and clearly states that this bait must not be used inside an operational food premises. Bait is poisonous and could be tracked onto food and/or food contact surfaces and possibly cause injury.
- Corrective Action(s): Remove all bait and bait stations from inside restaurant and clean all areas around where bait was placed to remove any bait residues.
- Pest control company arrive during inspection and removed all bait stations. Cleaning of areas where bait stations placed still needs to be done. Pest control company stated a 10% bleach solution must be used after the cleaning on all areas where mouse activity was found to destroy the phermone scent.

- Observation: Premise general sanitation has improved over previous inspection however, significant number of mouse droppings have been found in food storage areas and food equipment areas. Observed mouse droppings next to immersion blender, next to bowls used for washing produce at line cooler, in boxes of paper towel and on bags of grain/pulses in dry storage room.
- Corrective Action(s): Do another deep clean of premise, removing all mouse droppings. Areas of food build-up were missed from last deep cleaning, including (but not limited to), around mixer base, under deep fryer, under pizza oven, behind under three compartment sink, back corners of pizza box storage (under pizza dough prep area).

- Observation: Significant number of mouse droppings observed throughout the premise including on pizza pan shelves, behind chest freezers, along walls behind oven and coolers. Pest control company currently coming in every two weeks however there are no reports available to be reviewed. Ask pest control company to provide reports (if not already provided) after each service and keep on site for review.
