- The prep cooler was measured at 9 °C. Food items including pepperoni, meat, chicken, and cut vegetables were measured at approximately 10 °C. The operator was instructed to immediately move all food items to the walk-in cooler. The prep cooler must be repaired to maintain a temperature of 4 °C or below. All high-risk foods must be stored at 4 °C or below at all times.

- There were no paper towels at the handwash sink located in the main kitchen; the operator refilled the dispenser immediately. Handwash sinks must be fully stocked at all times.
