- Botox Cosmetic neuromodulator was not being used according to the manufactuer's instructions for use, namely:- reconstitution was done using bacteriostatic saline instead of preservative free saline.- reconstituted Botox cosmetic was stored in the refrigerator and labeled with a date, and no time (Sept 9).Please ensure MIFU is followed:- reconsitute with preservative free saline- once reconstituted, ensure it is used within 24 hours, then discarded. Label the date and time of reconstitution.
