      • ** - a build up of grime, grease, food was observed on the following items that includes but is not limited to: the counter cooler lids (top and underside), all buspans and lids, the rice container and lid, the milk crates, the upright freezer door, woks, pots, rice cooker inserts, and shelving.

      • ** - no mouse traps or pest control system was in place (required)** - no written pest records were available upon request (required).** - holes existed around plumbing in several places, please seal up the holes.

      • ** - dirty single use cardboard covering was observed on shelving in back kitchen. Please remove. All surfaces in a kitchen must be smooth, cleanable, non absorbent and durable.
    • 22. Are utensils and equipment maintained in a condition and a manner that ensures the safe handling of food?
      • ** - staff were placing dirty shoes on food shelving - please retrain staff not to place dirty objects on food storage surfaces. Please clean and disinfect the shelf.

      • Hot water tap at the hand wash sink in the back kitchen was not functional. ACTION REQUIRED: repair hot water tap on the indicated sink.
