    • 03. Are clients at this facility being protected from hazards (Critical)?
      • 1. Push pins were used in the toddler room. Push pins present a choking hazard and are not appropriate for use in rooms with children under 3 years of age. The push pins were removed during the inspection. Alternative methods that are safe and appropriate for the age group include tape over staples, double-sided tape, command hooks, velcro, magnetic sheet/sign holders.2. Several push pins were observed in the infant room, including on walls and the ceiling, and in proximity to cribs. Push pins present a choking hazard and are not appropriate for use in rooms with children under 3 years of age. Remove all push pins, particularly those located above sleeping, play, or activity areas where they could fall, and use alternative methods that are safe and appropriate for the age group.

      • 1. Primo bingo dabbers were noted in the preschool and toddler rooms. The manufacturer’s recommended age is 12+.The bingo dabbers were removed during the inspection. Ensure all toys and materials are age-appropriate to prevent potential safety risks.2. Staff personal belongings were stored in a cubby in the toddler room and were accessible to children. Personal items may contain potentially hazardous contents (e.g., medications). A staff member confirmed no hazardous items were inside their purse at the time of inspection; however, the purse was relocated during the inspection as a precaution. Ensure all staff personal belongings are stored out of reach of children at all times.

      • 1. A section of the ceiling in the preschool room, near the maintenance room, was water damaged. Staff reported a recent leak due to heavy rainfall. Repair and refinish the affected area to ensure the surface is smooth, intact, and maintained in a sanitary condition.

      • 1. The toy bed in the preschool room is in disrepair. The mattress material is deteriorating and crumbling into pieces. The mattress sheets looked stained and unclean. Repair or replace the damaged mattress and ensure all linens are clean and in good condition.2. Some foam play blocks in the preschool room were in disrepair.Discard any damaged foam play blocks.

      • 1. A torn shirt and damaged dress were observed in the toddler room. The damaged shoulder straps posed a potential entanglement hazard. The items were removed during the inspection. Ensure all play /dress-up clothing is regularly inspected and removed if damaged to prevent potential safety hazards.

      • 1. The OSC bathroom had an empty soap dispenser. Soap was replenished during the inspection.

      • 1. Baby room: There is a fridge and microwave right next to the diapering change mat. - Please move the fridge and microwave to prevent any risk of cross contamination. June 19, 2025: Discussion on site with the operator about potential resolution regarding this matter.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • 1. Washroom adjacent to the toddler room: A drywall screw was found on the bathroom countertop where it is accessible to children. Staff report it came off from the wall. This can be a hazard to children if ingested or handled.- The drywall screw was removed during the inspection.2. The cabinet storing chemicals inside the kitchen were not locked. - Staff locked the cabinet during the inspection. Please ensure this cabinet is locked at all times.
    • 06. Are proper protocols being followed in regards to client care?
      • 1. Baby room: There is a fridge and microwave right next to the diapering change mat. - Please move the fridge and microwave to prevent any risk of cross contamination. 2. Toddler washroom: Observed staff conducting diapering procedures. The staff member did not use soap to wash the child's hands. This was witnessed twice during observation. Only water was used to rinse their hands.The safety strap was not used to at the start of diapering procedures. Please ensure children are strapped at all times while on the mat.- Education was provided to staff during the inspection. Please ensure staff follow proper diapering procedures.

      • 1. Toddler room: A section of the baseboard is in disrepair. All walls need to be in good repair and easy to clean.- Please repair the baseboard.

      • 1. A clean and dirty laundry bin is not available for laundry.- Please have labeled clean and dirty bins. This can help organize laundry.

      • 1. The storage door located in the preschool room is missing a handle. The piece sticking out of the doorknob is at eye level for a child and can be a protrusion hazard.- Please replace the doorknob.2. Toddler room: There is a portable fan that had an accumulation of dust.- Please wipe down the fan and remove the dust accumulation.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • 1. There is improper storage of bedding noted in the facility.Preschool room: Some of the bedding is stored on the floor. The clean and dirty bedding is not separated.Before and after school: The bedding stored inside the closet are stacked on top of each other and touching. - Please separate and organize the bedding. Individual bedding should not be touching each other and should be stored in a sanitary manner.

      • 1. Carpet in toddler room is worn and stained in some areas. - Ensure that carpets are in good condition. Replace if necessary. 2. Preschool washroom: The floor mat in front of the sinks was dirty. - Replace

      • 1. Two dead cockroaches were found in a cupboard under the sink in the preschool washroom. - Clean the cupboard and monitor for any additional pests. Hire a PCO if more pests are found.

      • Some paint in toddler room was for ages 3 yrs and up. - Remove art products that are not approved for this age group.

      • Flooring was damaged (cracked/detaching) by the preschool washroom and storage room. - Ensure that the floors are in good repair and easy to clean. Surface around a floor vent in OOSC room was damaged . - Ensure that the surface is smooth and easy to clean. Left side of a counter in OOSC washroom was detaching. - RepairWooden shelves for diapering supplies were damaged and not easy to clean anymore in toddler washroom. - Ensure that the shelves are smooth and easy to clean. Gap in flooring behind a toilet in infant room. - Ensure that floor is smooth, moisture resistant and easy to clean. Vinyl tiles in a cupboard under the sinks were lifting.- Repair so the surface is smooth and easy to clean.

      • Carpets were stained in toddler and infant rooms. - Steam clean the carpets regularly. Foot operated garbage bin in toddler washroom was not working.- Repair so staff do not have to touch the lid after handwashing.

      • Preschool room: Linoleum flooring was damaged in several areas such as by the back wall between toilets and in the classroom (by cubbies, sand tables etc).- Repair the flooring. Preschool room: The back of the blue storage unit was damaged in the first cubby. - RepairPreschool room: Some sleeping mats were damaged and mended with tape. - Replace any damaged sleeping mats. Preschool room: A play table with a carpet on top was damaged and sand was accumulating in hard-to-reach areas. - Repair or replace.

      • Chemicals were stored under a sink in OOSC washroom. - Store all chemicals out of children reach.

      • 1. No liner was used under a child during diapering. - Use liner under each child during diapering. 2. A child's hands were not washed after diapering. - Wash children's hands after diapering.

      • 1. Dirty wiping cloths were hanging on bleach spray bottles. - Replace wiping cloths after each use. 2. Cupboards in washrooms were disorganized. - Clean regularly. 3. Hard-to -reach areas in washrooms were dirty. - Clean regularly.

      • A few dead cockroaches found on a ledge in the preschool room. - Thoroughly clean and disinfect all surfaces and hard-to-reach areas. Store all food in plastic containers with tight fitting lids.

      • Toddler room: A staff member did not wash hands after changing diapers. - Staff must wash hands after diapering.
