- 1. Push pins were used in the toddler room. Push pins present a choking hazard and are not appropriate for use in rooms with children under 3 years of age. The push pins were removed during the inspection. Alternative methods that are safe and appropriate for the age group include tape over staples, double-sided tape, command hooks, velcro, magnetic sheet/sign holders.2. Several push pins were observed in the infant room, including on walls and the ceiling, and in proximity to cribs. Push pins present a choking hazard and are not appropriate for use in rooms with children under 3 years of age. Remove all push pins, particularly those located above sleeping, play, or activity areas where they could fall, and use alternative methods that are safe and appropriate for the age group.

- 1. Primo bingo dabbers were noted in the preschool and toddler rooms. The manufacturer’s recommended age is 12+.The bingo dabbers were removed during the inspection. Ensure all toys and materials are age-appropriate to prevent potential safety risks.2. Staff personal belongings were stored in a cubby in the toddler room and were accessible to children. Personal items may contain potentially hazardous contents (e.g., medications). A staff member confirmed no hazardous items were inside their purse at the time of inspection; however, the purse was relocated during the inspection as a precaution. Ensure all staff personal belongings are stored out of reach of children at all times.

- 1. A section of the ceiling in the preschool room, near the maintenance room, was water damaged. Staff reported a recent leak due to heavy rainfall. Repair and refinish the affected area to ensure the surface is smooth, intact, and maintained in a sanitary condition.

- 1. The toy bed in the preschool room is in disrepair. The mattress material is deteriorating and crumbling into pieces. The mattress sheets looked stained and unclean. Repair or replace the damaged mattress and ensure all linens are clean and in good condition.2. Some foam play blocks in the preschool room were in disrepair.Discard any damaged foam play blocks.

- 1. A torn shirt and damaged dress were observed in the toddler room. The damaged shoulder straps posed a potential entanglement hazard. The items were removed during the inspection. Ensure all play /dress-up clothing is regularly inspected and removed if damaged to prevent potential safety hazards.

- 1. The OSC bathroom had an empty soap dispenser. Soap was replenished during the inspection.

- 1. Washroom adjacent to the toddler room: A drywall screw was found on the bathroom countertop where it is accessible to children. Staff report it came off from the wall. This can be a hazard to children if ingested or handled.- The drywall screw was removed during the inspection.2. The cabinet storing chemicals inside the kitchen were not locked. - Staff locked the cabinet during the inspection. Please ensure this cabinet is locked at all times.

- 1. Baby room: There is a fridge and microwave right next to the diapering change mat. - Please move the fridge and microwave to prevent any risk of cross contamination. 2. Toddler washroom: Observed staff conducting diapering procedures. The staff member did not use soap to wash the child's hands. This was witnessed twice during observation. Only water was used to rinse their hands.The safety strap was not used to at the start of diapering procedures. Please ensure children are strapped at all times while on the mat.- Education was provided to staff during the inspection. Please ensure staff follow proper diapering procedures.

- 1. Toddler room: A section of the baseboard is in disrepair. All walls need to be in good repair and easy to clean.- Please repair the baseboard.

- 1. The storage door located in the preschool room is missing a handle. The piece sticking out of the doorknob is at eye level for a child and can be a protrusion hazard.- Please replace the doorknob.2. Toddler room: There is a portable fan that had an accumulation of dust.- Please wipe down the fan and remove the dust accumulation.

- 1. There is improper storage of bedding noted in the facility.Preschool room: Some of the bedding is stored on the floor. The clean and dirty bedding is not separated.Before and after school: The bedding stored inside the closet are stacked on top of each other and touching. - Please separate and organize the bedding. Individual bedding should not be touching each other and should be stored in a sanitary manner.
