      • A large multi-colored play structure in the basement gross motor room was observed with several rips which make the surface difficult to effectively clean and sanitize. Ensure the indicated piece of equipment is repaired, replaced or removed.

      • Two plastic play cars in the basement gross motor room were observed to be in disrepair with sharp edges which can potentially injure children. Please ensure all equipment is maintained in good repair to help prevent potential injuries. The operator removed the indicated toys during inspection.

      • Single-use gloves were observed to be hanging on the handwashing sink in the infant room. Please ensure single-use gloves are used once and then discarded after use. The operator discarded the indicated gloves during the inspection.
