      • The facility did not have the illness records at the time of inspection. The facility must always maintain and document the illness records. Cleaning and Sanitation checklist were not found during the time of inspection.Facility must provide Cleaning and Sanitation checklist.

      • The facility used reusable- rags to clean the surfaces. The facility must not use rags for cleaning the surfaces as they are single use. Alternative solution is provided, such as use of paper towels.

      • Corrected: The bleach solution spray bottle present at handwashing sink was unlabeled and upon testing with chlorine strip, it was measured to be 0 ppm. Outstanding: The facility did not have the chlorine test strips. The bleach bottle must be labeled- staff corrected. Preparation of bleach solution- 2 caps of bleach in 1 liter of water.The bleach solution must be measured with the chlorine test strips (100 ppm).

      • The toy washing procedure was not followed accurately. The facility missed first step in toy washing (soaking in soap water).The facility was advised to always soak toys in soap water, rinse and then soak in bleach solution (100ppm).
