- ** - staff were observed touching dirty surfaces and phones with gloves on** - when PHI asked that contaminated gloves be changed out, staff went to grab fresh gloves without properly handwashing with soap and water in between glove changes. See guidelines on glove use provided with report.

- ** - contaminated cell phones (x2) were observed stored on a paper towel roll in the front and on the food preparation surface in the back room. DO NOT use cell phones during food preparation. Store the cell phones in areas that do not contaminate handwashing supplies or food contact surfaces or in an easy to reach manner that may lead to the contamination of food being prepared for the public.

- Scoops were observed stored inside the food containers with the dirty high contact surface (handle) in direct contact with the food ingredient. Please ensure that scoops are stored outside of the food containers to prevent contamination of the food item.

Please ensure that the grill is cleaned and degreased to prevent contamination and possibility of a fire from the grease build up.**-- Mold-like substance was observed on the 3- compartment sink. Please ensure the sink is cleaned properly and sanitized to prevent contamination with pathogenic organism during food processing.**-- The eating end of customer eating spoons were observed to be stored up and exposed for contamination. Please place the eating end down to prevent contamination.

Please ensure that all equipment in the food facility is cleaned thoroughly to prevent contamination during food handling.** Customer bathroom was observed to be unsanitary. The baseboard of the toilet requires thorough cleaning. Please ensure all surfaces in the washroom is cleaned and disinfected thoroughly to prevent contamination during food handling.
