    • 06. Are proper protocols being followed in regards to client care?
      • Proper diapering procedures were not followed. The staff member did not use a liner while changing a child's diaper. After the diaper change, the child was allowed to return to the play area without having their hands washed. The staff member then proceeded to handle the bleach bottle stored with other bottles, touching multiple surfaces and cleaning supplies without appropriate hygiene practices. The staff member needed to be reminded to wash the child's hands.Please re-train all staff on proper diapering procedures and ensure they are following the right procedure.

      • 1. 4–5 year old classroom: One of the toilet lids is taped. Please repair it.2. Toddler room: The paint on the diapering table is peeling, making the surface rough and difficult to clean. Please repair or replace the table to ensure the surface is smooth and easily cleanable.

      • 1. Toddler and 3–4 year old classrooms: Pushpins are being used on the walls. Please remove them, as they may fall onto the floor and pose a risk of injury to a child. Tape or sticky tack should be used instead.2. Young toddler room: The nearly 5‑foot‑tall cubby is unsecured and moveable. Please secure it.

The foam puzzle pieces in the toddler room has various bite marks along the edges and corners. 1. and 2. Please discard the damaged toys as they cannot be properly cleaned and disinfected.

      • Toys were being cleaned and sanitized in diapering sink/area in baby room.-Do not use diapering sink/area to clean and sanitize toys to prevent contamination. Please provide dedicated sink/area to clean and sanitize toys.
    • 22. Is the plumbing system adequate and being properly maintained (Non-Critical)?
      • Hot water temperature was measured at 24.5'C in baby and toddler's diapering/washroom sink.-Please increase and maintain hot water temperature between 38 - 43'C.
