- 06. Are proper protocols being followed in regards to client care?
- Proper diapering procedures were not followed. The staff member did not use a liner while changing a child's diaper. After the diaper change, the child was allowed to return to the play area without having their hands washed. The staff member then proceeded to handle the bleach bottle stored with other bottles, touching multiple surfaces and cleaning supplies without appropriate hygiene practices. The staff member needed to be reminded to wash the child's hands.Please re-train all staff on proper diapering procedures and ensure they are following the right procedure.

Tape or sticky tack should be used instead.2. Young toddler room: The nearly 5‑foot‑tall cubby is unsecured and moveable. Please secure it.

- 1. There were broken toys in the 3-5 year old classroom. The food toys (plastic baguette/bread loaf and white bottle) are damaged. 2. The foam puzzle pieces in the toddler room has various bite marks along the edges and corners. 1. and 2. Please discard the damaged toys as they cannot be properly cleaned and disinfected.
