      • Dirtied/soiled single use containers were observed to be stored in the same storage bin right beside new and unused containers. The used containers were all discarded immediately upon request.

      • Take out boxes filled with lettuce were stacked on top of each other with the bottom of each box directly touching the lettuce. Plastic wrap was used to separate each box and mixing bowls to prevent contamination.

      • Food debris spillage was observed in a storage bin. The storage bin was cleaned and sanitized immediately upon request.
