- Chlorine sanitizer was measured at 0ppm . Operator prepared a new solution when prompted. Ensure all sanitizer solutions are at the appropriate concentration to allow for optimal sanitization.

- Hand sink by the stove was observed to be blocked by a mop bucket and cleaning supplies. Operator removed when prompted. Ensure hand sinks are accessible at all times to facilitate proper handwashing.

- Chlorine sanitizer was 1000pm, which is higher than the approved concentration of 100ppm. Corrected during inspection. PHI coached Operator on mixing 100ppm Chlorine sanitizer.Action Required:Ensure sanitizer are kept at approved concentrations (100ppm Bleach or 200ppm QUATs or 12.5-25ppm Iodine, not higher as that increases the likelihood of contamination of food contact surfaces or lower as that prevents effective cleaning and sanitizing procedures.

- Some of the shelving inserts of the upright cooler and freezer were rusty. Repair or replace rusty or damaged inserts. Ensure all surfaces are made of material that is smooth, durable, impervious and easily cleanable to facilitate effective cleaning and sanitizing procedures. Correct by March 12, 2026, and email PHI photo evidence of corrective actions.
