    • 06. Do operators and personal services workers comply with the Personal Services Regulation and Standards and do they have the skills and knowledge to perform their duties in a safe and sanitary manner?
      • October 21, 2025: The registered nurse demonstrated a lack of knowledge regarding single use items and proper disposal of sharps. October 9, 2025: The registered nurse demonstrated a lack of knowledge regarding the cosmetic injectable products in use, associated manufacturer instructions, applicable legislation, and required infection prevention and control procedures.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • A bottle of opened sodium chloride, labelled as single use, was observed on the counter in room 9 following the treatment of a client. This item was disposed of at the request of PHI R. Reive.
    • 24. Are waste sharps disposed of properly?
      • Three used syringes, with caps on, were observed in room 9 following a treatment performed by RN Solange. One was observed left out on the counter, and two more were found in the drawers of a small organizing unit on top of the counter. When asked, the RN stated she had not yet cleaned up from the previous client but was planning to do so, and said the two in the drawers were not put there by her. All 3 were disposed of properly into the sharps container.

      • Two bottles of expired sterile saline were observed in the reprocessing room. Product expired in October of 2024. Both bottles were disposed of by PHI R. Reive.

      • Cosmetic injectable products, including fillers (brand names included Restylane, Revanesse, and Juvederm), were labelled as single-use. 16 partially-used syringes of filler were found opened and stored in cabinets, only one of which was labelled with a date of October 7, 2025.

      • Cosmetic injectable products, including fillers (brand names included Restylane, Revanesse, and Juvederm), were labelled as single-use. 16 partially-used syringes of filler were found opened and stored in cabinets, only one of which was labelled with a date of October 7, 2025.

      • Three single-use, prefilled syringes of filler (Restylane Defyne, Volyme and Refyne) were observed in storage with used needles still attached.

      • Cosmetic injectable products, including Dysport and fillers (brand names included Restylane, Revanesse, and Juvederm), were labelled as single-use. Three bottles of reconstituted Dysport, dated August 7 2025, October 1 2025 and October 7 2025 were found in the fridge partially-used. 16 partially-used syringes of filler were found opened and stored in cabinets, only one of which was labelled with a date of October 7, 2025.

      • Cosmetic injectable products, including Botox and Dysport, had been reconstituted with Bacteriostatic Sodium. Manufacturer's instructions for both products require the use of 0.9% sterile, preservative-free, saline.

      • Cosmetic injectable products, including Botox and Dysport, were being stored and used beyond the manufacturer's instructions of 24-hours post-reconstitution. Dates of reconstitution were written on the packaging by the operator, with dates ranging as far back as February 18, 2024. 23 total bottles of reconstituted Botox and 3 bottles of reconstituted Dysport were observed stored in the fridge beyond 24-hours. A single vial of Dysport, date of reconstitution August 7, 2025, was labelled "for office use".

      • Facility was in operation without having submitted a PSN. PSN was submitted at time of inspection at request of PHI.
    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Written procedures were not available at time of inspection. Please print out and complete the attached templates and keep on site for review by staff at all times.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Facility is not documenting sterility of single-use pre-sterilized items with client records. Lots and expiration dates must be kept in record for client for all pre-sterilized items.
    • 19. Is ultrasonic cleaning of reusable equipment being performed properly and when necessary?
      • Microdermabrasion tips require an ultrasonic machine to remove debris from hard to clean areas.
    • 20. Is reusable equipment disinfected properly?
      • CS20 was being reused without required record keeping or daily test strips. Purchase required test strips and begin keeping daily records per template provided OR cease reuse of CS20.
