    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • 1) Wooden toys throughout the facility have been worn off, had paint coming off or had cracks and were rough to touch.- Repair or replace the wooden toys.2) The toy bins throughout the facility have had dust and debris build up. -Clean and disinfect the toy bins.3) The grey cabinet in the pre-school 2 room had paint chipping off. - Repaint the cabinet such that it is smooth, non-absorbent to moisture and easily cleanable.

      • The diapering mats in infant room1, infant room 4, toddler room 2, have rips and therefore are not easily cleanable and absorbent to moisture anymore.- The operator replaced the diapering pads with new pads.

      • 1)The plywood on the ledge of handwashing sink in the preschool 2 room is coming off, exposing the wood underneath and there is not easily cleanable and is absorbent to moisture. - Repair/ reseal the plywood. 2) The ledge near the handwashing sink in the preschool 2 room is separating from the wall.- Repair the ledge.3) The paint on the back wall in the pre-school 2 room is chipping. - Repaint the wall.

      • The kitchen set, black and decker toy, and little people toy in the pre-school 2 room had dust and debris accumulated. - Clean the aforementioned toys.

      • Unlabeled sanitizer bottles were observed in almost all classrooms.-Please properly label all chemical bottles/containers to prevent against misuse.

      • Some sensory material containers/bottles in classrooms can be opened. The lids of these containers or bottles should be glued so that children do not have access to the content or liquid within.-Please ensure the lids of indicated containers are securely glued so that children do not have access to hazardous materials.

      • Used cleaning cloths were observed left on classroom surfaces.-Please remove and store cleaning cloths in a designated container immediately after use to prevent reuse and cross contamination of surfaces/equipment.

      • Dirty floor/area observed beside the sink in toddler 4 room.-Please clean the indicated area and ensure it is maintained in sanitary condition at all times.

      • Dirty play equipment/toys were observed in infant 2 room.-Please clean and sanitize the indicated toys, and ensure they are in sanitary condition at all times.

      • The faucets in preschool 3 washroom were wobbly and laden with layer of dirt around the sink and faucets.-Please fix the damaged faucets. Also, clean and maintain in sanitary condition: washroom faucets, hand washing sinks, and all high touch surfaces.
