- - In the pre-school room 4, the blind cord safety hold broke off during the inspection.Ensure that all the dangling cords are safely secured to prevent any injury.

(Age group 2.5 to 3 years).Operator promptly removed the toys from the play area.It is highly recommended to go through all the toys in the facility accessible to children aged 3 and less and remove any toys or arts/craft material that can pose choking hazard.

- During the inspection, the laundry room door was observed to be open. The violation was corrected at the time of inspection.To prevent unauthorized access and ensure child safety, doors to non-program areas—including mechanical rooms, janitorial rooms, kitchens, and laundry areas—must be kept closed and inaccessible to children at all times.

- An unlocked cabinet was observed in the pre-school 4 room and Toddler 2 room.Cabinets were locked during the inspection, by the staff members.

- The big fabric cushion in multiple rooms were observed to be visibly dirty with food residues and stains.Cushions were removed from the children's rooms and taken to the laundry room.Ensure that all surfaces in child are rooms are visibly clean.

- Chewed foam blocks were observed in the toddler rooms 8.Operator promptly removed those blocks from paly area during the inspection.

- 1) Multiple childcare toddler rooms either did not have the mouthed toys bins or the mouthed toy bins were not properly labelled.Concerns were discussed with the staff member, Ensure that all the toddler and infant rooms are provided with mouthed toy bins.2) Mat in the gym area was showing the signs of disrepair and needs to be repaired or replaced.

- Electrical receptacles were exposed and accessible to children in the diaper changing area in Infant Rooms 1 and 3.Please ensure that electrical receptacles are equipped with safety covers, and cords are removed from the area.

The use of raw flour as an ingredient poses a potential risk of contamination with E. coli or Salmonella, which can spread through handling and play.The operator promptly discarded the playdough and will switch to using starch products or purchasing premade products.2) A metal tap with sharp edges was accessible on the play kitchen unit located in the Preschool 3 Room.The operator promptly removed the play unit from the room.

- Undercounter coolers used to store milk were installed underneath the diaper changing areas located in the following areas but not limited to Infant Room 1 and 2.Please relocate the diaper changing area away from any food storage to prevent cross-contamination and the spread of disease.

- First aid kits were missing CPR masks and alcohol swabs.Please ensure all first aid kits throughout the facility are fulling stocked with essential supplies including CPR masks and alcohol swabs.

- The carpets throughout the facility in each room were observed to be covered in stains, crumbs, dried sauce, and debris.Please deep clean all contaminated carpets throughout the facility. Ensure children are eating on non-carpeted floors to prevent contamination of soft material.

- The diaper changing pad located in the Toddler 6 Room was observed to be soiled in dried playdough, dirt and debris.The operator promptly replaced the changing pad. Please ensure that all changing pads throughout the facility are thoroughly cleaned and disinfected.

- 1. Infant 3 room: Push pins were being used to hang items in the wall. The pins can be a potential hazard if swallowed by a child. The push pins were removed during the inspection.Avoid using push pins and stick to safe alternatives.

- 1. Diapering mats in Infant 1, 2, and 3 are in disrepair. The mats are ripped making is difficult to clean effectively. The operator was already aware of the issue and mats have been ordered.New mats will be arriving on August 12th.2. The diapering mat in infant 3 room had build-up of food debris under the mat. The operator cleaned under the mat during the inspection.Please ensure staff remove the mat when cleaning to ensure all debris is removed and wiped down thoroughly.3. There was a chair in infant 3 room with build-up of dried food debris. It was wiped during inspection.Please ensure staff are wiping and sanitizing the chairs and table thoroughly.4. The cribs in infant 3 room had several fingerprints on the plastic panels and one of the mats had debris build-up. The mats and cribs were wiped down during the inspection.Please ensure staff are wiping down the cribs on a regular basis.

- 1. The bouncer chair in infant 2 had build-up of food debris on the seat cover. The operator stated the bouncer chair is currently not being used. The cover was removed during the inspection to be washed.2. Infant 3 room: The window sill had build-up of dust and debris. The operator wiped down the window sill during the inspection.Please remind cleaning staff to wipe down the windows.3. The arm rest for one of the wooden chairs in infant 3 were loose which could be a potential pinching hazard. The chair was removed from the room.

- 1. Chlorine bottles (100 ppm and 1000 ppm) did not have a proper labels to help identity the contents inside. - Some of the bottles were labeled during inspection. Please ensure all chemical bottles have proper labels.

- 1. The laundry bins are unlabeled, and staff are using one container for the soiled and clean laundry. - Please have two separate laundry bins for the clean and soiled laundry. Have labels on the containers.

- 1. Infant 3: Brown debris noted around the diapering area. It was noted on the cabinets and on the wall.- Please clean and sanitize the area.

- 1. Preschool 3: There were toys being immersed with water inside the toy washing basin. There was no chlorine in the basin when tested. Staff report a bottle of 100 ppm chlorine was poured inside the basin but it was diluted even more.- Please ensure there is 100 ppm chlorine when sanitizing the toys.

- 1. Play materials that cannot be effectively cleaned and disinfected easily were being used during the outbreak (ex. books, plush toys, mesh fabric for dance sessions) in several rooms. - Please do not use play materials that cannot be effectively cleaned and disinfected during outbreak. The items were being removed during the inspection.2. The rugs have not been removed in several of the rooms. - Please remove rugs to allow more easily cleanable floor surfaces during outbreak. The rugs were being removed during the inspection.3. Staff report the mop heads are cleaned on a monthly basis. - Please clean and disinfect mop heads on a daily basis even when outbreak is declared over. After a day of use or as needed.- Additional mop heads may need to be purchased.4. Preschool 3: There was a shared water table being used. - Staff were advised to close the water table until the outbreak is over.5. Large fabric pillows noted in preschool 2 room. This item will be difficult to clean and sanitize between use.- Please remove the pillows until the outbreak is over.
