      • Mice feces were observed in the on the bottom shelves of the packaged, bread display.
    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • 1) There's a large gap under the second/interior sliding door at the entrance/exit. --recommend installing another door sweep to prevent mice from entering after the exterior doors.2) There were gaps at various structures/pipes/walls, etc throughout the store. These gaps and holes can be hiding spots for mice and other pests.--seal up all gaps, holes, cracks, etc.

    • 23. Is the facility maintained in a clean and sanitary condition?
      • The following areas had accumulation of debris/food:-bakery production area shelves had crumbs, seeds, etc. Thoroughly clean all shelves in the bakery area and ensure items are moved to complete the cleaning. -packaged baked goods shelving (customer area). There were crumbs and mice droppings on shelves. Also, the container for box storage at self-serve baked good display was dirty. Move pallets and/or rolling racks to clean under the stationary items.-under the produce table (customer area) had many rotting fruits and debris. Clean and remove. This area needs to be cleaned on a daily/weekly basis as it appears it is easily forgotten since it's hard to reach because of the compressor in the area. -the back of the house produce prep area was dirty with debris and old produce (around the compost bins).

    • 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
      • Bakery Dept - No hot water at handwash sink on far end.Cheese Island - No running water at handwash sink. Soap dispenser not dispensing properly. Meat Dept - Inadequate soap dispenser at handwash sink. Paper towel dispenser at back area sinks empty. Deli Dept - Paper towel dispenser empty at handwash sink. All handwashing sinks must be equipped with both hot and cold running water, soap in suitable dispensers and paper towel in dispensers.This is a critical issue and must be addressed immediately.

      • Pipe below triple sink in produce cutting area was badly leaking when water was turned on. Pool of water on floor below sinks. Leaking fixtures must be fixed.

    • 22. Are utensils and equipment maintained in a condition and a manner that ensures the safe handling of food?
      • Approximately half the bulbs in the hot holding display case for cooked chicken were burnt out. One chicken probed measured exactly 60 degrees Celsius internally.Replace burnt out bulbs to help ensure hot foods remain at required temperature for the max length of time they are stored in unit.
    • 23. Is the facility maintained in a clean and sanitary condition?
      • Surfaces and / or equipment dirty in various areas, specifically:Bakery Dept - Build-up of dirt and debris on floor of walk-in freezer.Dirt present on walls in far end area.Handles of cupboards used to store icing had caked on food debris.Deli Dept - Floor and shelf in meal prep area were dirty.Meat Dept - Build-up of black / green substance present on floor and lower walls of dishwashing room. Condenser fans / vents were dirty and dusty.Produce Dept - Floor below shelves, and some shelves were dirty. Pallets used to store food in walk-in cooler were dirty. Dairy Cooler - Dust and dirt present on ceiling and upper walls.All noted areas require cleaning.Ensure all areas are maintained in a clean and sanitary condition.
