Shelves in room 101, 109, and 107 were replaced during the inspection.Please repair/replace the shelf in room 120A to ensure the surface is smooth, non-absorbent, and easy to clean.2. The small couch in room 104 ripped making it difficult to clean.Please repair/replace the couch to ensure the surface is smooth, non-absorbent, and easy to clean.

      • A table with worn surface exposing raw wood was observed in one of the 3-6 years classrooms.-Please repair or replace the indicated table to ensure proper cleaning and sanitizing.

      • 1. A torn diapering mat was observed in the baby’s room.-Please replace the damaged mat to ensure the surface can be properly cleaned and sanitized, maintaining sanitary condition.This was corrected during the inspection.2. Numerous materials were stored on the diapering surface.-Please remove all items from the diapering surface and surrounding area to prevent contamination and allow for effective cleaning and sanitizing.This was corrected during the inspection.3. The bleach solution used for cleaning/sanitizing the diapering surface was measured at 50 ppm.-Please dilute and ensure adequate concentration of bleach solution of 100ppm and daily monitoring with suitable test strip.This infraction was corrected during the inspection.4. Micro Kleen surface disinfectant was diluted to clean/sanitize classrooms surfaces. No test strip available onsite to verify its concentration.-Please provide suitable test strip.

      • A table with worn surface exposing raw wood was observed in one of the 3-6 years classrooms.-Please repair or replace the indicated table to ensure proper cleaning and sanitizing.

      • The hot water temperature at the handwashing sinks in the baby and toddler rooms was measured at 33°C.-Please increase and maintain the hot water temperature at these sinks within the recommended range of 38°C to 43°C to support effective handwashing process.

      • Some children's water bottles were observed without name tags in baby and toddlers' classrooms.-Please ensure all children's bottles are labeled with their names.

      • Several cupboards under sinks throughout the facility that are used for chemical storage do not have child locks. Install child locks or relocate chemicals.
    • 06. Are proper protocols being followed in regards to client care?
      • Care giver was wiping diapering surfaces with lysol wipes. This is not a suitable disinfectant for diapering. Facility switched to accel wipes during inspection. Recommend 100ppm chlorine.
    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • Facility is using PCS200 as a disinfectant. This product has a 10 minute contact time. Choose a product with 1 or less minute contact time.

      • 1. Build up of debris and grim on toys, mats, stains on upholstery couches/chairs, hard surface chairs, hallway walls near kitchen, step stools throughout the facility. Ensure these areas are cleaned thoroughly.2. Cutting boards used by children to cut an portion snacks in room 106 are stained and gouged. Replace cutting boards.

      • Stay in place and wooden toys are not being cleaning with soapy water weekly. Ensure toys are cleaned with soap and water at least weekly.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • Bedding in several rooms are stored in and out of bags stacked on each other. Ensure bedding is stored separately to prevent potential spread of pathogens.
