- 1. A torn diapering mat was observed in the baby’s room.-Please replace the damaged mat to ensure the surface can be properly cleaned and sanitized, maintaining sanitary condition.This was corrected during the inspection.2. Numerous materials were stored on the diapering surface.-Please remove all items from the diapering surface and surrounding area to prevent contamination and allow for effective cleaning and sanitizing.This was corrected during the inspection.3. The bleach solution used for cleaning/sanitizing the diapering surface was measured at 50 ppm.-Please dilute and ensure adequate concentration of bleach solution of 100ppm and daily monitoring with suitable test strip.This infraction was corrected during the inspection.4. Micro Kleen surface disinfectant was diluted to clean/sanitize classrooms surfaces. No test strip available onsite to verify its concentration.-Please provide suitable test strip.

- Several cupboards under sinks throughout the facility that are used for chemical storage do not have child locks. Install child locks or relocate chemicals.

- Care giver was wiping diapering surfaces with lysol wipes. This is not a suitable disinfectant for diapering. Facility switched to accel wipes during inspection. Recommend 100ppm chlorine.

Ensure these areas are cleaned thoroughly.2. Cutting boards used by children to cut an portion snacks in room 106 are stained and gouged. Replace cutting boards.
