      • Lots of storage materials observed stored on the floor in one of the upstairs rooms.-Please store these materials above the floor to ensure the room can be cleaned and maintain in sanitary condition.

      • One uncovered light fixture still remains in the toddler classroom.-Please install appropriate covers or replace the bulbs with shatter-resistant alternatives. This will help mitigate potential hazards in the event of bulb breakage.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Dirty vent observed in the toddler classroom.-Please clean and maintain in sanitary condition.

The diaper/changing mat in the baby room has numerous small holes or pores, making it difficult to clean and sanitize effectively.-Please replace the diaper mat with one that has a smooth, non-absorbent surface that is easy to clean and disinfect, in order to maintain proper hygiene standards.2. Lots of storage materials observed stored on the floor in one of the upstairs rooms.-Please store these materials above the floor to ensure the room can be cleaned and maintain in sanitary condition.

      • 1. Uncovered light fixtures were observed in several classrooms and in the kitchen area.-Please install appropriate covers on the affected light fixtures or replace the bulbs with shatter-resistant alternatives. This will help mitigate potential hazards in the event of bulb breakage.2. Sensory bottles were observed to have multiple layers of adhesive tape, making them difficult to clean. Additionally, openable sensory bottles were observed in the baby room.-Please seal all sensory bottles using non-toxic glue to ensure they remain securely closed and hygienic for use.

      • 1. Worn leather chair was observed in baby room.-Please repair or replace the indicated chair to ensure sanitary condition.2. Dirty Carpets were noted in several classrooms.-Please clean and regularly deep clean the carpets to maintain sanitary.3. Dirty vents and light fixtures were observed in the preschool classroom.-Please clean and maintain in sanitary condition.4. Dirty toys and containers were also noted in toddler and preschool classrooms.-Please clean and sanitize classrooms toys and containers.

      • Children's blankets in preschool classroom were observed folded together.-Please store each child’s blanket separately and ensure they are clearly labeled with the child’s name to maintain hygiene procedures.

For example, bleach solutions exceeding 1000 ppm were noted.-Please label all sanitizer bottles with the chemical name and concentration.If using bleach solutions at 1000 ppm or higher, surfaces should be wiped afterward with clean water and disposable towels or wet paper towels to reduce chemical residue.4. Lots of storage materials observed stored on the floor in one of the upstairs rooms.-Please store these materials above the floor to ensure the room can be cleaned and maintain in sanitary condition.

Fish tank with sitting water and debris left in the 4-5 room. Remove fish tank.4. Several items under the baby's room bathroom sink. Area is cluttered. Ensure this is organized and easily accessible.

      • 1. Operators are washing toys every few weeks. Ensure hard surface toys are washed with soapy water and disinfected weekly.2. Dress up clothes and soft surface toys are washed monthly. Ensure soft surface toys are laundered and disinfected weekly.

      • 1. The lid for the sand table in the toddler room has rough sharp edges. Remove or replace. 2. Several toys, a foam floor mat and chairs throughout the facility have a build up of grim. Ensure these are thoroughly cleaned. If they cannot be cleaned discard.
