      • 1. Hippo and Lions Rooms: Individual chalkboards are damaged along the edges, with exposed raw wood surfaces that are not cleanable.Repair or replace the damaged chalkboards to ensure all surfaces are smooth, intact, and easily cleanable.2. Elephant room: There are toys (Wooden lacing apple toy and the cardboard puzzle) that have damaged surfaces and are not easily cleanable.The toys were removed during the inspection.

    • 06. Are proper protocols being followed in regards to client care?
      • 1.Bulk supply of Vaseline was contaminated during the diapering procedure. Staff were scooping directly out of the jar after removing the diaper. Please ensure all items from a bulk source are portioned with a single use item or prepared prior to diapering procedure to prevent cross contamination.Operators discarded the Vaseline.2. diapering mat was not thoroughly wiped down. Mats must be disinfected and wiped down entirely after diapering.Operator was educated about cleaning and disinfecting protocols.

      • The diapering station was dirty underneath the mats in the cheetah and lion's room.Please clean and disinfect the area underneath the diapering mat so that it is in a clean and sanitary condition.

      • The mats used in the diapering area in the Cheetah and elephant room had rips on the covers. Please replace/repair the mats so the surface is smooth, waterproof, and easy to clean.

      • 1. A pair of scissors were stored on the diapering change table. - Staff removed the scissors during inspection. Please ensure hazards are removed from children's reach.2. Staff store their personal belongings in lockers located inside the classroom which are accessible to children. There is a potential for staff to be storing items like personal medication or other hazardous items. - Please add locks to ensure belongings are inaccessible to the children.

      • 1. There were a couple of push pins in the wall. Please avoid using push pins.- The pins were removed during inspection.

      • 1. The wall by the window in the Baby Room was damaged, and a section of paint was missing. ** Please repair the damaged wall and apply a proper paint finish so the surface is made smooth, non-absorbent to moisture, and easily cleanable. 2. The upper door trim to the Baby Room was missing and raw porous wood/ rough uneven materials were observed.**Please replace the missing upper door trim to the Baby Room. Update on July 19, 2024: extended completion date to August 30th. Contractor is in place for the repair of the wall and sink counter replacements.

Ensure it is properly labelled. 3. A bottle of bleach was stored in an unlocked cabinet (accessible to children) by the food handling station sink in the Monkey Classroom.** Please relocate the bottle of bleach, so it is inaccessible or, install a childproof lock for the cabinet door.4. The area was very messy as well (bowl of chilli on the counter and other items) were blocking accessibility to the handwashing area/ soap/ paper towel dispenser etc. **Please ensure the dedicated handwashing sink and all supplies are accessible for staff and the area kept clear to encourage handwashing. **This was corrected during the inspection**

      • July 5, 2023 inspection:1. Lion room: Staff was observed not disinfecting diapering surfaces between clients. As well as not preforming handwashing.Please review the correct diapering procedure. 2. There was debris under the change pads in all rooms.Ensure that staff are cleaning and disinfecting under the diaper change pads on a frequent basis.
