- Observation: Operating permit observed to be displayed in a non conspicuous location, in corner of dish washing area.
- Corrective Action: Move operating permit to a conspicuous area in view of customers.

- Observation: Vent hood for pizza/salad prep area observed to have a significant buildup of grease during inspection. Grease buildup may result in contamination of prepared foods due to dripping.

- Observation: Dirty dishware stored directly on the floor during inspection beside three compartment sink in dish washing area.
- Corrective Action: Dish ware is to be kept at last 6" off of the floor to prevent any cross-contamination between food contact and non-food contact surfaces.

- 301 - Critical Equipment/utensils/food contact surfaces not maintained in sanitary condition [s. 17(1)]
- Observation: (CORRECTED DURING INSPECTION) Quartz sanitizer in the sanitization buckets was measured at 100 PPM. Used wiping cloth was observed laying on the food preparation surfaces.
- Corrective Action: Operator refilled the buckets with fresh sanitizer and the concentration was maintained at 200 PPM. Inadequate sanitizer concentration may lead to improper sanitization of the food contact surfaces in turn may lead to cross contamination of the other food products being prepared on the same surfaces.
- 304 - Premises not free of pests [s. 26(a)]
- Observation: Inspection conducted on 12th July around 4:00 PM. Mice droppings were observed in the bar area, on the stairs towards the basement, food preparation area in the basement and in walk-in cooler. It was seen that a mesh was placed at the walk-in cooler door to prevent the mice from entering. Operator mentioned that they cleaned the food storage racks in the dry food storage area and professional pest control company has visited the site on 11th July and set up bait traps in the facility.
- Corrective Action: Operator was told to submit a copy of invoice for the pest control company (1st Defence) and was instructed to properly clean and sanitize the entire facility and get rid of the mice droppings. Operator was told to install a proper seal to the basement entrance door and walk-in cooler door. Operator was given two weeks to address the issues and a follow up inspection will be conducted.
- 305 - Conditions observed that may allow entrance/harbouring/breeding of pests [s. 26(b),(c)]
- Observation: Unused kitchen equipment were found right in front of the hand washing sink in basement preparation area.
- Corrective Action: Operator must ensure that only the equipment used for food processing must be kept in the kitchen and unnecessary equipment, tools should be removed from the premises. Presence of unnecessary items may hinder proper cleaning and sanitizing the areas inturn lead to trapping food residues which may attract pests. It was observed that the clutter is making it difficult for the employees to wash their hands.

- Observation: (CORRECTED DURING INSPECTION) It was observed that several spray bottels with contents inside were not labelled.
- Corrective Action: Operator labelled them during the inspection
- 311 - Premises not maintained as per approved plans [s. 6(1)(b)]
- Observation: Flooring in the Kitchen area, basement food preparation area is not in a good condition and seen worn out in few places.
- Corrective Action: All floors in the premises must be smooth, durable, easily cleanable and non-slip and impervious to liquids. Operator reported that there is a plan in place to renovate the flooring in the facility in January 2023. Worn out flooring can hinder proper cleaning and sanitizing in turn hold onto food residues which may attract pests into the premises.
