    • 08. Is this facility properly constructed (Non-Critical)?
      • 1) The walls adjacent to the sleeping mats in the toddler room and above the handwash sink in the infant room were in disrepair, exposing raw drywall.Please repair and repaint the affected wall surfaces.2) The flooring transition surrounding the handwash sinks located in the infant room were in disrepair, exposing sharp edges.Please repair the floor transitions.3) The ceiling tile located in the kinder room was bowing, potentially becoming a falling hazard.Please repair or replace the ceiling tile to prevent the risk of injury.
    • 22. Is the plumbing system adequate and being properly maintained (Non-Critical)?
      • The caulking around both handwash sinks located in the infant room were in disrepair.Please remove and replace the old caulking with a new bead of silicone.

    • 10. Is a properly supplied first aid kit provided and readily available?
      • - The alcohol swabs and gauze were found to be expired in all of the first aid kits.*Please ensure to keep a record of such items and replace them as required to maintain proper stock and ensure safety.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • - One of the diapering tables in the infant room was visibly dirty under the mattress.*Staff cleaned and sanitized the table during the inspection.**Ensure that all surfaces are maintained in a clean and sanitized condition.
    • 30. Are workers washing hands when appropriate and/or in a proper manner?
      • - A staff member was observed handling two different children without washing hands during the diapering procedure in the Toddler room. The staff wore gloves on one hand, discarded the gloves after changing the first child, and proceeded to the assist the second child without washing hands. The area was sprayed with bleach, wiped down with paper towels, and the glove was changed, but proper handwashing was not performed.*The staff was informed about the significance of handwashing and reminded that gloves do not replace the need for handwashing.**Hands must be washed regardless of glove use between diapering procedures to ensure proper hygiene and prevent cross-contamination.

    • 06. Are proper protocols being followed in regards to client care?
      • 1. There was a tower fan on the diapering change bed.The fan was removed at the time of inspection. Ensure the diaper change surface is not used for any other activity aside diapering. 2. The diapering change bed was covered with fabric linen. Upon discussion with the staff, it was discovered that the linen is changed at the end of the day. The fabric linen was removed at the time of inspection. The diapering change surface must be sanitized between each use.
