      • There were holes in walls and baseboards in the front service area, cabinets, around plumbing drains, and in the office area.-repair all holes to prevent mice harborage. All surfaces must be smooth, durable, non-absorbent, and easily washable.

      • There were holes in walls and baseboards in the front service area, cabinets, around plumbing drains, and in the office area.-repair all holes to prevent mice harborage. All surfaces must be smooth, durable, non-absorbent, and easily washable.

      • 2 signs were covering the public Closure Order Notice on the front door (10:20am, April 21, 2026).-remove signs. Do not cover the public Closure Order notice.

      • 1) Mice feces observed behind and under equipment, shelves, etc in the front service area, decorating area, and storage areas. -clean and disinfect all areas that have mice feces.-Hire a pest control professional to help resolve the mice issue **UPDATE: there were approx 5 traps, without bait in open areas. Hire a pest control company to resolve the mice infestation as they will help set-up traps in proper locations.2) There appears to be a mouse nest behind the ice cream machine in the front serving area.-move ALL equipment, shelves, tables, coolers, etc. Clean and disinfect surfaces contaminated with mice feces. All items must be moved so activity can be identified and monitored.

      • 1) There were gaps at the back door which would allow mice and other pests to enter premises.-replace weatherstripping and replace any additional parts needed to eliminate the gaps under/around the door.2) There were large gaps at the front door; it was previously repaired with a flimsy piece of plastic.-obtain proper door sweeps to prevent pests from entering the food premises.

      • Openings were observed at the back door of the restuarant. Action required: Seal all openings to prevent entry of pest into the facility.

      • Flash freezer had a buildup of ice around the door, and water was leaking on the floor. Operator indicated that the door gasket was not the correct size. Please repair.
