- Observation: Slices of pizza stored in the hot holding unit measured at 48C. At least 60C or above is required to properly hot hold pizza. This is a repeated violation. Operator was time tracking the pizza. According to the corporate food safety plan submitted, tracking was not part of the food safety procedure.
- Corrective Action(s): Repair or exchange the hot holding unit to ensure hot food can be held at 60C or above. Submit updated food safety plan for proper time tracking. Repeating this violation in the future will result in enforcement actions.

- Observation: Broken and inappropriate dispensers, with no handles, were used and stored within bulk food containers.
- Corrective Action(s): Dispensers were removed. Operator must purchase appropriate dispensers, with handles, and store in a fashion where the handles do not come into contact with food to avoid cross-contamination. Also, when any food contact items are broken, they must be discarded to prevent any potential physical contamination.
