      • Observation: Minor rodent (rat) activity observed on site with chewed food boxes and nesting material in dry kitchen storage unit. Facility has increase pest control management and prevention measures. Staff have sealed any possible entry ways into the kitchen. Pest activity has been significantly reduced.
      • Corrective Action(s): Clean up old rodent activity, disinfectant surfaces and monitor for new activity. Enlist service from pest control company as needed to reduce rodent population if they are in the building. Correction required in two days.

      • Observation: Old rodent nesting material and feces observed in hard to clean areas behind counters and in corners of storage areas. Cleaning and sanitizing is required. At the time of inspection, food service area and food source is protected from contamination.
      • Corrective Action(s): Ensure to clean and sanitize with bleach old rodent activity areas, especially hard to clean areas. Monitor for new pest activity and contact pest control service if required. Correction required in one week.

      • Observation: Dishwasher is leaking and requires warming up before it can reach required 71C at the plate for sanitizing. The temperature monitor guage is also broken.
      • Corrective Action(s): Ensure this unit is in working order and can reach the sanitizing step at 71C at the plate daily. In the interim while awaiting for repair, ensure to check internal temperature of unit with a dishwasher safe temperature monitoring probe with maximum temperature logging.
      • Alternatively, if the unit is not working, the last sanitizing step can be completed manually in a clean sink with 200 ppm QUAT sanitizer or 200 ppm Chlorine sanitizer then air dried.
      • Correction and repair required in two weeks.

      • Observation (CORRECTED DURING INSPECTION): Food contact surface sanitizer not available at the time inspection. At least one food grade sanitizer at the right concentration is required during food service.
      • Corrective Action(s): Ensure a food grade sanitizer with a properly labeled container or bucket in use. Concentration of Chlorine solution required is 100 to 200 ppm Chlorine or QUATS at 200 to 400 ppm. Log concentration of chemicals tested daily for records.

      • Observation: Minor crack in hand sink in the kitchen with small amounts of building material debris observed. Ensure the debris is cleaned up so it does not contaminate food surfaces or food products.
