- Observation (CORRECTED DURING INSPECTION): No sanitizer was detected from the 3-compartment sink sanitizer dispenser at the time of inspection. No sanitizer available in produce cutting area.
- Corrective Action(s): Manager stated they would contact technician to service sanitizer dispenser. In the interim, use another sink / functional sanitizer dispenser for cleaning and sanitizing equipment in this area (quats must be 200 ppm or per product label specifications for food contact surfaces). A labeled sanitizer spray bottle with 200 ppm quats was provided for the produce cutting area during the inspection. Ensure equipment (e.g. cutting boards, knives) are re-sanitized.

- Observation: - One package of dry noodles located in first aisle was observed with chew holes. Note: Package was beyond best before date and may have been missed during cleaning / restocking.
- - Few dry droppings (may be old) observed on shelves in first aisle. Affected areas were shown to manager at time of inspection.
- - One mouse observed in a tin cat trap located next to the loading bay doors.
- Corrective Action(s): Packaged noodles removed. Ensure affected areas are thoroughly cleaned and sanitized to remove all droppings today, and all dry storage areas / shelves are checked and re-cleaned and sanitized.
- Contact pest control to remove mouse and reset trap. Ensure bay doors / back door remain closed when not in used, and follow pest control recommendations to further minimize pest activity.

- Observation: Evidence of rodent activity observed in retail area and back area.
- 1 rodent observed in a trap located to the right of the receiving doors.
- Two packages of food observed with chew holes in retail area. Spilled food observed near damaged packages.
- Rodent droppings observed on floor located on the left side of the grocery walk-in freezer, and on some shelving in retail area. Areas were pointed out to management during inspection.
- Corrective Action(s): Have pest control technician reset trap. Date to be corrected by: June 8, 2024.
- Damaged packages of food were discarded. Ensure product is being checked for signs of damage/contamination on a daily basis.
- Ensure areas affected by rodents are properly cleaned and sanitized today, and continue to work with pest control and monitor for new activity. Clean and sanitize on a daily basis.

- Corrective Action(s): Seal this hole to effectively prevent entry of pests in this area. Date to be corrected by: June 8, 2024. Monitor and identify any other potential entry points, on an ongoing basis.

- Corrective Action(s): Manager adjusted dispenser/line and quats was made available (200 ppm quats). Note: Spray bottles had proper concentration of quats (200 to 400 ppm). Ensure any equipment/utensils that were sanitized using quats directly from the dispenser are resanitized as a precaution.

- Corrective Action(s): Ensure facility is maintained in a pest free condition. Remove all dead rodents and rodent droppings. Sanitize all affected areas using a 1:10 bleach solution and monitor for increased activity. Ensure facility is maintained in a clean and sanitary condition at all times and doors are kept closed when not in use for deliveries.
