Quats sanitizer was initially detected, however quats concentration continued to decrease as floor cleaner was being pulled through the dispenser. Manager stated overnight cleaning staff may have swapped the chemical lines last night.
- Corrective Action(s): Manager removed chemical lines, rinsed, and flushed the sanitizer line. Quats sanitizer was detected at 200ppm after flushing.
- Contact the chemical supplier / technician to ensure chemical dispenser is in good working order and chemicals are changed / properly flushed. Manager to discuss issue with all staff.
- Note: Quats was available at 200ppm in labeled spray bottles, and manager stated they were filled yesterday. However, as a precaution, quats sanitizer bottles were emptied. 100ppm chlorine was provided in buckets with wiping cloths to be used in the interim until the chemical dispenser is re-serviced, and surfaces were re-sanitized.

- Corrective Action(s): The following items were discarded:

- - 1 10 kg bag flour
- - 2 40 lb bags rice
- Note: Other food is stored in pest resistant containers with lids. No signs of contamination. Exterior of containers cleaned and sanitized during inspection. Going forward, ensure all dry storage food items stored in pest proof containers (e.g. pest proof containers with tight fitting lids). Operator obtained additional storage bins with lids at time of inspection.

- Corrective Action(s): Bottles emptied, refilled at dispenser, and measured 200 ppm quats. Ensure sanitizer is maintained at concentrations required for safe and effective sanitizing of food contact surfaces (e.g. 200 ppm quats).

- Observation (CORRECTED DURING INSPECTION): Aluminum foil observed lining wire rack in left prep cooler (at cook line). Foil is showing signs of deterioration.

It was stated that a pest control technician services the premises; however, operator unable to provide most recent pest control report.
