- Observation: Mouse activity (droppings) found in the following areas:
- -Electrical room in the basement (significant droppings)
- -Basement dry storage room (droppings around room perimeter)
- -Floor around the dough mixer
- -Floor and window sill around the pizza prep table
- Corrective Action(s): 1) Perform enhanced cleaning around the pizza prep table and dough mixer daily (ie. every night before end of shift)-remove all flour/dough from surfaces to eliminate food attractants
- 2) Ask pest control technician to take enhanced pest control measures in the electrical room
- 3) Ensure staff keep doors to the building exterior and basement corridors closed at all times (except when receiving deliveries).

- Observation (CORRECTED DURING INSPECTION):
- Observed staff wiping their hands on their aprons, which was very stained, and not washing their hands after answering the phone and using the ordering screen. Bacteria can and love to grow on damp, warm materials (aprons, clothes) and surfaces with food debris.
- Corrective Action(s):
- Retrain staff on the proper and frequent washing of their hands. Staff SHALL wash hands when changing tasks; prior to handling food; and/or when their hands become soiled. Do NOT place the broom infront of the hand sink, as it makes it difficult/discourages staff from washing their hands.

- 1) The interior walls and mechanical parts of the ice machine have mould on them. The mould may fall/touch the ice which patrons could ingest it.
- 2) The ledge below the front windows in the pizza dough area has accumulated flour and dough on it. This will attract pests and is visible to patrons outside.
- Corrective Action(s):
- 1) Clean the interior walls and mechanical parts of the ice machine with hot soapy water, rinse with clean hot water, and then sanitize with hot water and bleach. Ensure to turn the ice machine off first and empty all the ice out.

- 1) Wire racks in the walk-in-cooler.

- 4) Floor beneath the wire racks in the walk-in-cooler.
- 5) The downstairs staff washroom floor is dirty. This was mentioned in the previous inspection.
- Corrective Action(s):
- All mentioned areas require to be washed with hot water & soap, and sanitized. Increase the frequency at which these areas are cleaned to prevent build up. The lowest rack, of the wire shelves in the walk-in-cooler, shall be raised 6" off the floor so that staff can get a broom under the rack to sweep up spilled food on a daily basis. Otherwise, staff will need to move the whole rack to clean underneath each day.

- 3) The handle on the ice bin, below the pop dispensing machine, is broken and covered in tape.

- Observation (CORRECTED DURING INSPECTION):
- The bleach sanitizer buckets storing cloths had [Cl2] of 0ppm. Staff explained that a 'little' bleach is added to the bucket and the solution is changed twice a day (11am and 5pm). The bleach sanitizer strength must be maintained at 100ppm in order to kill and germs that the cloth may pick up. This violations was mentioned in past inspections.
- Corrective Action(s):
- Add 1tsp. of bleach to the bucket and fill with warm water to make the correct mixture strength. The solution MUST be changed every 2 hours. Retrain staff on the correct process to make the sanitizer mixture, and to change it every 2 hours. Staff made new sanitizer solution during the inspection.

- Observation:
- 1) The ice making machine has mold/mildew on the inner bin walls and the mechanical parts. Mold/mildew will contaminate the ice.
- 2) The beer lines and metal box in the walk-in-cooler has mold and mildew built up on it. Mold and mildew can contaminate food being stored in the walk-in-cooler.
- 3) The staff washroom floor and outer surface of the waste bin, in the basement, has accumulated dirt and debris. This dirt can transfer to staff uniforms when they use it.
- 4) The window ledge and floor under the front pizza preparation table has flour and crumbs. This food will attract pests.
- Corrective Action(s):
- 1) Empty the ice machine bin, wash and sanitize the bin and mechanical parts to remove the mold/mildew. Increase the frequency at which this is done as no mold shall grow between cleanings.
- 2) Wash and sanitize the bear lines and metal box. Increase the frequency at which this is done as no mold shall brow between cleanings.
- 3) Scrub the floor and waste bin surface to remove the black dirt and debris. Increase the frequency at which this is done so no dirt accumulates.
- 4) Clean the crumbs and flour from these surfaces. Pull the table(s) away from the wall to access the floor beneath to clean. Increase the frequency at which this is done.
