      • Observation(s): One bottle of a cleaning chemical was noted to be in a room and accessible to persons in care. This was immediately put away when discovered.

    • R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
      • Observation(s): Throughout the facility minor repairs are required: window trim is missing or coming off in one person in care's bedroom and above one door way in another person in care's suite, there are holes in the walls in one person in care's bedroom, walls throughout the facility have minor chips and gouges in the drywall and paint, and the siding adjacent to the back deck is still in need of repair or replacement as a result of the deck repairs being completed. The laminate flooring upstairs extending into the kitchen, dining space, hallway, and main entry is still in need of repair or replacement. This was noted on the inspection report of July 8, 2016. Throughout the facility cleaning is required: cobwebs on sprinkler system heads, dirt build up and smudges on walls and door frames and along the floor against the mouldings. This was also noted on the inspection report dated July 8, 2016. It was also observed that left over construction materials are present on the back deck, which could be considered a trip hazard for persons in care.

    • 4.2 Are Medication Administration Records accurate, kept on site and are adverse events documented and reported?
      • Observation(s): Upon review of Medication Administration Records (MARs), it was noted some are missing initials or notations to indicate whether medication was administered as ordered or not. This is a reoccurring infraction noted on the inspection report dated August 21, 2014.

    • 6.3 Does the facility demonstrate appropriate outbreak prevention and control measures?
      • Observation(s): One person in care has a personal fridge in their suite. This fridge is not being monitored for temperatures at this time to ensure person in care's food and beverages are stored safely.

    • 7.1 Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
      • Observation(s): During the inspection the following was observed: 1) flooring upstairs in the kitchen, dining space, hallway and entry is in need of repair or replacement as the surface is worn in places and the laminate planks are coming away from each other leaving some gaps where dirt and debris could build up; 2) there are some holes in the walls where previous thermostats or hardware from the old heat exchange system used to be; 3) there are minor paint chips and gouges in the walls throughout the facility; and, 4) the door used to secure the furnace is needing repair to ensure the furnace is inaccessible to persons in care. It was also observed that: a) the corner seam on the kitchen counter top is separating slightly and there is a section of the laminate broken off the front edge - both exposing the material underneath causing the countertop to no longer be non-porous; b) some carpets have dark stains on them; c) in one person in care's rooms there are two electrical outlets that have multiple appliances plugged into them causing concern for fire or electric shock; and, d) throughout the facility it was observed there is dirt build up and smudges on walls, along the walls on the floors, and on doors.
