- There is evidence of a cockroach infestation in the facility. Live and dead cockroaches (10-15) were observed on glue traps mostly in the dishwashing and hand sink areas.- The facility is being monitored and treated by a licensed Pest Control Company.- Store food in pest proof containers with tightfitting covers to prevent potential pest contamination. - Do not allow garbage, food particle and debris to accumulate in a manner that would facilitate the harborage or proliferation of pest.- Seal/caulked all openings noted between steel panels and walls throughout the kitchen to prevent pest harborage.- Keep the floor and walls in the dishwashing area dry as much as possible. Cockroaches are highly attracted to water.- Check all plumbing fixtures in the facility to ensure there are no plumbing leaks.

- The handwashing sink at the bar was not easily accessible for handwashing.Remove all items blocking the hand washing sink. All handwashing sinks in the facility must be free of any items and easily accessible for handwashing.
