- Preempt CS20 disinfectant (expires May 28, 2025) was available.The owner could not say exactly when the solution in use was last changed, and the sterilization log was not filled out correctly, with results missing for some months.The log that was last used had all 30 dates filled out with "Apr" and "pass" checked off.There were two test strip bottles, both expiring February 28, 2026, however one was labeled as opened on October 24, 2024. This bottle of test strips must not be used past 3 months of opening as directed by the manufacturer.The owner tested the existing Preempt CS20 solution with the AHP7 test strips and result was a "pass".Corrective actions taken:- Preempt CS20 solution was discarded. The remaining amount of disinfectant from the bottle was poured out into the disinfection tray. Day 14 of this solution coincides with the expiry date of the disinfectant (May 28, 2025).- Disinfectant solution was tested with AHP7 test strips, results were a "pass".- New HLD log sheet was completed with the assistance of the PHI.- Since this was the last of the high level disinfectant, the owner will have to acquire more disinfectant to be used after May 28, 2025.

- - Oxivir TB intermediate level disinfectant was available. One of the bottles was expired since May, 2024. - PSW were using alcohol antiseptic to clean and disinfect manicure tables and chairs.Corrective actions taken;- Going forward, ensure a disinfectant with a DIN is used for cleaning and disinfection of equipment and surfaces. Prodcuts that were currently available in the nail salon include, Oxivir TB (1 minute contact time), and Lysol wipes (4 minute contact time). Follow the manufacturer's instructions for use.

- Eyebrow tinting services is now being offered. Proper notification to AHS was not provided.

- Written procedures could not be presented to the PHI upon request.Written procedures for cleansing skin is required for the following services:-manicure-pedicure-waxingWritten procedures for accidental skin cuts or punctures is required.Written procedures for sink decontamination is also required if the reprocessing sink is being used for staff handwashing.

- Operator and workers are not following the requirements of the Personal Services Regulation and Standard. All personal services workers are required to review and ensure compliance with the current Personal Services Regulation and the Personal Services Standards. New legislation is proposed for June 30, 2025.

- The nail duster brush at the first manicure station closest to the cash register was visibly dirty. This is a porous/uncleanable tool. Clean using soap/water, isopropyl alcohol or hydrogen peroxide after each client use. Do not use disinfectant.

- 1. The swivel canister used to store cosmetic products used for tinting services was visibly stained/soiled. Clean and/or replace this container. Clean equipment must be stored in clean areas.

- 1. The glass dapper jar used for eyebrow tinting services was visibly soiled and/or stained. Clean the glass dapper jar to remove staining and/or discard and replace with a new jar.

- 1. High-level disinfectant logs (HLDL) are not being properly maintained. Missing logs for October and previous logs are not properly being filled out.2. PSW's are not following high-level disinfectant label instructions as required. PHI was told that tools are being soaked 15-20 minutes. When using PreEmpt CS20, tools are required to be soaked for 20 minutes. 3. Improper indicator test strips for the PreEmpt CS20 are being used. Proper indicator test strips are missing. If using PreEmpt CS20, use AHP7 indicator test strips. If using PreEmpt HLD5, use AHP2 indicator test strips. Obtain proper test strips.4. A larger disinfectant tray is needed. Buy a plastic/glass tray with a lid. Ensure the metal foot files can be fully soaked.5. The metal rotary bits are being soaked in glass jars filled with isopropyl alcohol at the manicure stations. Ensure the metal rotary drill bits are being disinfected in high-level disinfectant.

- 1. The soap petals allegedly used for pedicure services do not contain a proper label. The original/bulk supply must be labelled with product identity, manufacturer, directions for use and/or list of ingredients. Obtain a proper label.2. The waxing pots are missing lids which could contaminate the bulk supply. Obtain lids.3. Cosmetic products used for pedicure services have been pre-dispensed into tiny plastic containers with lids. Ensure they are properly labelled with its contents.
