      • The handsink in the boys' bathroom was not working. There was an alternate handsink nearby available for use in the interim. Please repair sink.

    • 06. Are proper protocols being followed in regards to client care?
      • 1. The infant room (yellow) had no bleach solution present for diapering. Operator was asked to make bleach solution immediately. Please ensure bleach solution is present in each room for diapering, as disinfection is crucial to preventing the spread of disease. 2. No test strips were present to test the concentration of the bleach solution. Please purchase. Bleach solution should be remade at minimum every day. 3. The bleach solution in the pink room was measured above 100 ppm, which is too strong. Above 100 ppm is not safe without a rinse step, and should only be used in an outbreak.

      • 1. The floors in the blue room had glue residue from tape on them, which was accumulating dirt and debris. Please remove tape and tape residue so floors can be properly cleaned.2. The wallpaper in the blue room behind the book shelf was peeling off and the surface underneath was in disrepair. Please ensure walls are smooth, impervious to moisture and easy to clean.

      • Used cleaning cloths were seen throughout the facility. Please ensure dirty laundry is stored in an appropriate receptacle. If the cloths are intended for re-use for cleaning purposes, please ensure they are stored in a chlorine solution of appropriate concentration, out of reach of children.

      • All rooms were missing paper towel during the inspection. Operator submitted photos of newly purchased paper towel the following morning. Please ensure the facility always has paper towel available for staff and children.

      • Peeling paint was observed on the walls in the green room, preschool room and toddler room. Please repaint these damaged areas.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • All handwashing sinks were missing paper towel. The child bathroom adjoined to the preschool room was missing soap. The operator submitted photos of newly purchased soap and paper towel the following morning. Please ensure there is always soap and paper towel available at all handwashing sinks as these supplies are critical for handwashing and preventing the spread of illnesses.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • Toys were observed floating in soapy water in the sink in the baby room. The operator stated bleach had been added to the water, but 0 ppm chlorine was measured after testing. The operator was educated on proper toy washing and sanitizing procedures and asked to re-wash and sanitize the toys.Please inform all staff of proper toy washing procedures. Toys must be first washed with soap and warm water to remove visible dirt/grime, rinsed with fresh water, then submerged in a bleach/water solution at a concentration of at least 100 ppm for at least 2 minutes. Do not add the bleach to the previously used soapy water, as soap can negatively interact with the chlorine in the bleach, rendering the bleach less effective at killing bacteria.
