- The following pest control concerns were noted:1. Area under the counter in the kitchen along the south wall (where the commercial tea/coffee urn & microwave is stored) has been cleaned.REQUIRED ACTION: consult with your pest control operator for cockroach monitoring and treatment purposes for this area as needed.2. Mouse droppings were observed in the following areas: --In the cupboard under the handwash station in the kitchen (March 5,2025: this area is still in disrepair and requires cleaning. This entire vanity/station is to be replaced, as per the owner. Due date: April 15, 2025).ACTION REQUIRED: Consult with your pest control provider regarding recommended mouse treatment.3. Mar. 5, 2025: all previously identified holes in walls have been sealed-up, and the back door closes flush with the ground when it is pulled completely shut. Ensure that the door is always pulled entirely shut so as to prevent the ~1 cm gap was observed along the base of the door (latch side)ACTION REQUIRED: Monitor the entire facility for additional holes and structural concerns.

Due date: April 15, 2025.The base of the cupboard below the designated hand wash station in the kitchen was in poor repair due to water damage; it was no longer smooth and washable. Water is leaking from the pipes inside the cabinet below the hand sink. ACTION REQUIRED: refinish the indicated surface so it is smooth, washable, & impervious to moisture. Repair leaking plumbing.

- The following pest control concerns were noted:1. Cockroach frass observed under the counter in the kitchen along the south wall (where the commercial tea/coffee urn & microwave is stored). A live cockroach was observed in this area. Several (>10) dead cockroaches were observed in the drawer. ACTION REQUIRED: conduct a deep clean of this area. Consult with your pest control provider regarding recommended cockroach treatment. 2. Mouse droppings were observed in the following areas: --In the cupboard under the handwash station in the kitchen --In the cupboard under the handwash station in the bathroom --On the floor along the south wall of the hallway from the kitchen to the bathrooms/public areaACTION REQUIRED: conduct a deep clean of the above noted areas. Consult with your pest control provider regarding recommended mouse treatment.3. The following conditions support the harborage of pests: --Observed a ~5cm x 10 cm hole in the west wall by the designated hand sink. --Observed a ~7x7 cm hole in the north wall by the upright cooler stainless-steel cooler. --Observed a ~4x4 cm hole at the base of the south wall of the hallway from the kitchen to the bathrooms/public area --The back exterior door was not tightly sealed; ~1 cm gap was observed along the base of the door (latch side)ACTION REQUIRED: seal the above noted area to prevent the entry and harborage of pests. Monitor the entire facility for additional holes and structural concerns.

- -It was observed that the staff member's personal belongings were stored along with the cleaned utensils in the reusable utensil storage section.Action taken:-Staff was instructed to store all the personal items separately in the designated staff area.

- -Cooked goat meat was observed sitting at room temperature, measured at 29C. -Staff member confirmed that the cooked meat was unknowingly left at room temperature. Action taken:-The goat meat was discarded during inspection due to time and temperature abuse (meat sitting at room temperature for more than 2 hours). -Time and temperature abuse concept was discussed with the staff.
