- Observation (CORRECTED DURING INSPECTION): Cutting board that was observed to be washed, rinsed, and sanitized did not allow for proper air drying to occur. Operator dried cutting board using paper towel. Food item that was to be cut on cutting board was then lined with parchment paper to prevent potential food contamination.

- Observation (CORRECTED DURING INSPECTION): *Repeat Violation* <200 ppm QUATs concentration detected in the three-compartment sink at the start of the inspection. Staff member indicated the solution was diluted since the chemical was irritating their hands.
- Corrective Action(s): - Staff refilled the sanitizer solution in the three compartment sink and 200 ppm QUAT concentration was detected.
- - Instructed staff that if the sanitizer is causing irritation, then a different staff member should be in charge of the dishwashing. Refer manufacturers label for minimum concentration to sanitize food contact surfaces, utensils and equipment to get rid of harmful bacteria and germs.

- Observation (CORRECTED DURING INSPECTION): < 150 PPM QUAT detected from the sanitize basin of the 3-compartment sink that was set-up during the inspection. No sanitizer container was set-up (only water available in the sanitize container) during the inspection.
- Corrective Action(s): **Issue corrected at the time of inspection** Staff refilled the sanitize basin and sanitizer container during the inspection. There must be adequate concentration of QUAT available (amount can be verified as per manufacturer's label) for it to sanitize food contact surfaces and utensils and equipment to get rid of harmful bacteria. Operator must ensure that the staff adheres to using a food-grade sanitizer on food contact surfaces when required. If staff has issue with working with this type of sanitizer, then they need to consult their head office or think of an alternate approved food-grade sanitizer to use.

Observed a staff filling out a pail of carrots with water using the hand washing sink, and the pail full of water stayed in the hand washing sink basin. As a result, the same staff was washing hands with a large pail of water occupying the basin. The pail was removed at this time.

- Observation (CORRECTED DURING INSPECTION): Observed spongy non-slip lining materials that were lining the shelves above the 3 compartment sink which stores cleaned and sanitized blender jars. The lining materials was water logged from water dripping from blender jars. The shelves above the 3 compartment sink was slightly slopped. Without these non-slip linings, jars would fall down.
- Corrective Action(s): Clean and sanitize the non-slip linings every 4 hours as per your manual dishwashing protocol. The lining materials must be removed permanently when the shelves are no longer slopped. Correct this by: Jan 21, 2022.

- Observation: Observed an ice machine in the additional storage area where each end of the door rails were held by large paper clips. As a result, the ice machine door had a permanent small opening (approximately 5cm gap) facing upward. The manager cited that without the clips, the door will just fall out of the rails and the clips were preventing from it. There was no drop ceiling right above the ice machine either. Paper clips were removed at this time and the door was fully shut at this time.

Food products were discarded. Staff member claim that the unit was operating at a temperature of 4C or below when the facility first opened.
