      • Observation (CORRECTED DURING INSPECTION): Sanitizer bucket had no detectable quats sanitizer.
      • Corrective Action(s): Sanitizer bucket emptied, rinsed, and refilled at the three-compartment sink with 200ppm quats. Counters re-sanitized. Ensure sanitizer is available (bucket(s) and/or spray bottle(s)) and is maintained at the required concentration for effective sanitizing (e.g. 200ppm quats). Change as often as needed to maintain correct concentrations. Note: A sanitizer log for sanitizer checks/changes every two hours is available but has not been filled in today.

    • 302 - Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
      • Observation (CORRECTED DURING INSPECTION): - Sanitizer bucket had no detectable sanitizer (quats).
      • - Sanitizer at three-compartment sink was slightly detectable, at less than 100ppm quats.
      • - Vegetable chopper was stored with dried on food debris in the blades.
      • Corrective Action(s): - Solutions discarded. Sanitizer must be maintained at safe and effective concentrations to kill/inactivate potential pathogens (required: 200ppm quats, or per product specifications for food contact surfaces).
      • - Staff initially opened the sanitizer valve only partially (approx. 45 degrees), which resulted in low sanitizer concentration. Staff then fully opened the sanitizer valve (90 degrees) and the correct concentration was dispensed (200ppm quats). Staff should be confirming sanitizer concentration with appropriate test strips.
      • - Surfaces and equipment (e.g. blender jugs) resanitized.
      • - Vegetable chopper soaked; to be re-washed and sanitized.

      • Observation (CORRECTED DURING INSPECTION): - Cooking oil spray is stored in the same cabinet along with cleaning chemicals.
      • Corrective Action(s): - Cooking oil spray relocated. Ensure all food is stored away from chemicals to protect from potential contamination.

      • 304 - Premises not free of pests [s. 26(a)]
      • Observation: - Mouse droppings observed on the premises. Refer to violation code 306 for affected areas.
      • - Note: No droppings observed in food areas, however, one open container of foil wrap appeared to be contaminated. Discarded.
      • Corrective Action(s): - Premises must be deeply cleaned and all areas affected by rodent droppings must be sanitized. Date to be corrected by: February 3, 2026.
      • - Have pest control re-service the premises.
      • - Provide the last two pest control reports to the undersigned EHO for review. Date to be completed by: February 3, 2026.
      • - Ensure all single use packaging/containers/utensils are covered/sealed and protected from potential contamination at all times.

      • Observation: - Dust, debris, and mouse droppings observed on floors under equipment, including but not limited to, freezers, coolers, and racks. This includes the front and back storage areas. In addition, a few droppings were observed in other areas including the hot water tank, on pipes near the fruit freezer, and some shelves.
      • Corrective Action(s): Ensure all areas of the facility are deeply cleaned and sanitized, including all counters, under equipment, under coolers/freezers/racks, and shelves. Areas affected by mouse droppings should be disinfected (e.g. 1 part bleach to 9 parts water). Remove any equipment not needed for business use. Date to be corrected by: February 3, 2026.

      • Observation (CORRECTED DURING INSPECTION): Scoop handles observed in direct contact with frozen fruit.
      • Corrective Action(s): Ensure scoops are stored in a manner so that the handles do not contact the food.

      • Observation: Rodent droppings observed in hard to reach areas, including floors (particularly under equipment and hard-to-reach areas), lower shelves of dry storage area, and around the hot water tank.
      • Corrective Action(s): Ensure all areas affected by droppings are cleaned and sanitized today. Ensure surfaces are checked on a daily basis and cleaned and sanitized as needed. As a precaution, ensure counters/prep areas are sanitized daily before opening, in addition to regular cleaning and sanitizing throughout the day as per your sanitation plan.

      • Observation: Several opening/gaps that may permit pest activity were observed in walls and ceiling, particularly where pipes and wires enter. Staff took photos during inspection.
      • Corrective Action(s): Ensure all holes and gaps that may potentially permit pest entry (e.g. 1/4" or larger) are effectively sealed. Steel wool is one example of a material that may potentially be used to seal holes and gaps, other materials (wire mesh, FRP, wood, etc.) may be used provided that they are resistant to pest entry. You may consult with pest control. Check all areas of the premises to locate/identify and seal areas that may permit pest entry. Date to be corrected by: July 18, 2025.

      • Corrective Action(s): Ensure all equipment is properly cleaned and after each use to prevent potential cross contamination of foods. Equipment may require additional presoaking in order to loosen old food debris. Cutter/press was washed and sanitized at the time of inspection.
