      • - Cleansing of the skin was not being done prior to a service that may involve contact with mucous membrane, or that will puncture or may potentially break skin. Ensure cleansing is done.

      • - Porous or uncleanable equipment (Nail files) is not being discarded after each use. Equipment classified as porous or uncleanable must be discarded after use in one personal service on one client.The operator voluntarily discarded used nail files.

      • - Contaminated equipment is not adequately separated and identifiable from clean equipment. The operator removed all used and contaminated equipment from the service area. The operator was also advised to have separate containers for storing clean and contaminated re-usable tools in order to prevent contamination of clean equipment.

      • -The high-level disinfectant being used by the operator was expired. The operator was directed to stop taking clients until a high-level disinfectant was acquired. The operator acquired an HLD during the inspection.

      • - High level disinfectants are not being tested and verified daily using proper test strips. Ensure the high level disinfectant is tested daily using the test strips specified by the disinfectant manufacturer.- Disinfection records were not available. Ensure records are kept of: test dates, whether the solution passed/failed, when the disinfectant was changed and the worker’s initials who performed the activity.
