- 06. Are proper protocols being followed in regards to client care?
- (1) Children's hands were not being washed in the Toddler room. Children could not reach the sink. There was no stepping stool at the sink for children to climb to reach the sink. During the inspection, a stepping stool was brought to the sink and staff started washing children's hands. Ensure handwashing is done on a regular basis in all rooms (before and after meals, after diapering, after outdoor play, etc) for children and staff.
- 06. Are proper protocols being followed in regards to client care?
- (1) The diaper change mat in the preschool room/transition room was torn and the surface could not be properly cleaned and disinfected. Ensure diapering mats are in good repair, so that they can be easily cleaned and disinfected.(2) The diaper change mat in the preschool room1/transition room was being stored inside the wall mounted diaper change station. This is not a proper storage place for the diaper mat. Ensure diaper mats have a designated storage (such as a wall hook) so that they can be kept clean and are able to dry between uses and to prevent contamination between other surfaces.(3) There was dirt and debris on the handle groove of the diaper change station in the toddler room. Ensure the entire diaper change table is properly cleaned and disinfected. This includes the handles, security belts and exterior surfaces of the wall mounted diaper change station.
- 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
- (1) Observed reused cloth hand towels. Was told that cloth towels were being used for children to dry their hands and that each child had their own hand towels that were being kept in the cubbies. Towels were being washed once a week. This was not acceptable as the towels were not within easy access, there were not sufficient towels available for each child to have their own (77 children) and the towels were not being washed frequently enough. Ensure a proper method of hand drying is available and used.(1) The staff accessible washroom hand wash basin could not be easily accessed due to the amount of miscellaneous items that were being stored in the washroom. Ensure the washroom sink can be easily accessed at all times to ensure staff are properly washing their hands after using the facilities.(2) There were no paper towels in the dispensers at the hand sinks throughout the facility. Ensure all paper towel dispensers are adequately supplied with paper towels at all times.(3) Hand soap bottles were being refilled and some were dirty along the inside rim of the bottle. Ensure soap bottles that are refilled are washed and dried prior to being refilled to prevent the growth of mold and mildew inside the bottles.
- 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
- (1) Used cleaning cloths were observed on top of the paper towel dispensers and other surfaces throughout the facility. Ensure cleaning cloths are used only once or are kept in a sanitizing solution between uses. If cleaning cloths will continue to be used, ensure there is a sufficient supply of clean cloths. Recommend having bins labeled for clean cloths and bins labeled for used/dirty cloths in the facility.(2) One of the feeding chairs in the infant room was dirty with old stuck on food debris on the seat. Children had not had their first meal of the day at the time of inspection. Ensure feeding chairs are cleaned on a regular basis. Recommend adding this task to the cleaning checklist. At this time, this was not one of the tasks listed on the infant room cleaning checklist.
- 21. Is the plumbing system adequate and being properly maintained (Critical)?
- (1) There was no hot water at the preschool room 1/transition room hand sink. The maximum water temperature was measured to be 26°C.(2) There was no hot water at the Infant room hand sink. The maximum water temperature was measured to be 24.5°C.(3) There was no hot water at the preschool room hand sink. The maximum water temperature was measured to be 22.7°C
- 23. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Critical)?
- (1) Loose blankets were being used to cover the mattresses in the cribs. These were not tight fitting and could pose a hazard if they come loose. Ensure tight fitting sheets only are used to cover the mattresses.
- 25. Are there adequate storage areas sufficient for the operation of the facility?
- (1) Hooks were missing in the preschool room 1/transition room. Some children's bags were being kept directly on the floor under the area where wall hooks were missing. Ensure there are sufficient hooks available for all the children's belongings.(2) A staff jacket was being kept on the play clothes rack in the toddler room. The jacket was stored amongst the play clothes. Ensure there is a designated storage place for staff personal items that is out of the reach of children. (3) Shoes were left on the carpet in front of the door leading to playground in the OSC side. This is a tripping hazard. Ensure the is adequate storage for all children's personal items including outdoor shoes.(4) Baby car seats were being kept directly on the floor in the mechanical room. Ensure children's personal items are stored in a designated safe and clean space.
- 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
- (1) The accessible washroom, next to the kitchen, was filled with miscellaneous items. There was a large shelving unit with the top shelf stacked with boxes of small appliances, extra cots were stacked on the floor next to the toilet, black bags were kept on the shelves, toys were piled at the entrance to the washroom. The hand sink was not accessible, and the washroom floor could not be easily cleaned and disinfected.(2) There was no paper towel dispenser for the preschool room washroom. Ensure a proper dispenser is installed for the preschool room washroom and that it is properly stocked at all times.
