      • Observation: Infestation of cockroaches throughout the kitchen remain. Sticky traps are full of cockraoches and not being replaced when full. Dead cockroaches are not being clean and removed from areas that have been sprayed.
      • Corrective Action(s): Clean and sanitize the following areas with dead cockroaches. Remove all sticky traps and replace them with new ones when they are full. Pest control company comes in every 2 weeks for spraying. Maintain general cleaniness in areas of infestation and increase cleaning frequency of the area.

      • Observation: Dishwasher does not reach >71C on the plate surface. Multiple runs done on the dishwasher and temperatures ranging from 56C to 65C.
      • Corrective Action(s): Conduct manual ware washing for all plates. Dishwasher can be used for wash and rinsing of dishes, Bleach sanitizer to be made in the 2 compartment sink for sanitizing purposes. Keep this method until dishwasher is able to maintain consistent temeprature of >71C on palte surface.

      • Observation: Few coachroaches crawling on and inbetween the dishwasher unit. No visible sticky traps seen being placed. Large build up of black residue seen behind the dishwasher.
      • Corrective Action(s): Coachroaches like warm and moist environments, clean out the following area and hire a professional pest control company to look at the issue. Clear out any unused cardboard boxes and items to eliminate potential harborage of pests such as coachraoches.

      • Observation: Gaps seen underneath the dishwasher units where piping come out of the wall and floor. Multiple areas along the window still has broken tiles/dry wall where the inside of wall is exposed.
      • Corrective Action(s): Repair/fill all gaps cause by damaged tiles/dry wall and plumbing fixtures.

      • Observation: Large accumulation of grease/debris seen at the following areas:
      • 1. Behind the wall along the dishwasher and on the dishwasher
      • 2. Inbetween and behind the stove top/ paneer cook top and deep fryers of the cooking area
      • 3. Food debris along the walls of the cooking area
      • 4. Hood vents have build up of grease
      • 5. Food accumulation and food debris in the dry storage areas
      • 6. In the walk in cooler along the walls and floors.
      • 7. behind shelving units in the dry storage area and the main kitchen preparation area.
      • Corrective Action(s): Clean and sanitize the following areas listed above and establish a routine cleaning schedule for these areas.

      • Observation: Dishwasher unable to reach 71C. Ran dishwasher several times and temperatures of 65-69C obtained.
      • Corrective Action(s): Repeat Violation:
      • Dishwasher must be able to achieve a temperature of >71C on the plate surface consistently to ensure dishes are being properly cleaned and sanitized. In the mean time, dishwasher can be used for washing and rinising of dishware. All dishware must be sanitized in a bleach solution located at the 2 compartment sink until dishwasher can be fixed.

      • Observation: Dishwasher (high temp conveyor) measured 61 C at plate after multiple attempts. As a banquet hall the proper function of the dishwasher is critical to minimizing risk to public health and ensuring dishes are safe to use.
      • Hard surface santizing to be confirmed at re-inspection.
      • Corrective Action(s): Service unit to make sure it can quickly reach proper operating temperatures and minimum 71 C can be reached at the plate.
      • This unit must be functional prior to next event and must be verified. Email service tech report and provide confirmation pic that unit is getting to proper temperature.

      • Observation: Cockroaches observed on sticky traps and on floor of kitchen, especially in storage room. Cockroaches were dead and the traps looked very old. Cockroaches like warm moist food environments and can come in via cardboard boxes.
      • Corrective Action(s): Get Pest Control technician in for assessment and to provide recommendations. Follow advice of Pest Control technician recommendations. Provide report to this office for review. Address the accumulations of food, moisture and remove cardboard boxes as soon as you can from the premises.

      • - walls around food prep areas;
      • - walk-in cooler floor and walls;
      • - Under dough mixer and work and storage shelves;
      • Corrective Action(s): Clean up the kitchen and secondary storage areas as discussed.
      • Follow up inspection in three weeks, but send pictures section by section as the kitchen is cleaned.

      • Observation (CORRECTED DURING INSPECTION): No sanitizer solution was prepared at the time of inspection.
      • Corrective Action(s): Staff mixed a fresh bucket of bleach-water solution and placed cloths into bucket once the issue was brought to their attention. Sanitizer was detected at 200ppm of chlorine.
      • *Ensure sanitizer is readily available for sanitizing food contact surfaces. Ensure chlorine of sanitizer is 100-200ppm.

      • Observation (CORRECTED DURING INSPECTION): All handwash stations in the kitchen were missing liquid soap and paper towel.
      • Corrective Action(s): For the handwash station in the center of kitchen, staff refilled the paper towel dispenser and has a temporary liquid soap pump available at the sink.
      • For the handwash station by the kitchen entrance, staff has napkins available at the sink for temporary use. Liquid soap is to be supplied for the handsink.
      • Handsink at the bar in dining area must be supplied with liquid soap and paper towel as well.

      • Observation (CORRECTED DURING INSPECTION): All handwash stations were not equipped for adequate hand hygiene
      • Corrective Action(s): Frequent and proper hand hygiene is critical to ensure safe food processing/handling. Make sure to fully supply handwash stations and maintain them to allow regualr and easy acess for hand hygiene.

      • Observation:
      • 1. Multiple containers of food stored in walk-in cooler were not covered.
      • 2. Plastic plate with no handle (used for scooping) was stored in the container of fried chips by dry bulk bin area.
      • 3. Interior of microwave was noted to be soiled and covered with food debris.
      • 4. The light fixtures located outside the walk-in cooler and above the ice machine is not covered, with the light bulb exposed.
      • Corrective Action(s):
      • 1. Ensure food stored are covered and protected from contamination
      • 2. Staff replaced the plastic plate with a scoop with handle for the noted container of food. Ensure the handle does not contact the food when stored inside the container. [CORRECTED]
      • 3. Clean the interior of microwave and maintain the equipment in sanitary condition.
      • 4. Ensure all light fixtures in the kitchen is covered to prevent potential contamination of food

      • Observation: Accumulation of dirt and food debris observed throughout the premises. The noted areas include but not limited to:
      • 1. Along the perimeter of the kitchen floor

      • 3. In the walk-in cooler, under the shelves. And the walls, which were noted with black stains..
      • Corrective Action(s): Remove debris and clean the noted areas and maintain premises in sanitary condition.

      • Observation: 1. Various sauces, meats, sweets and other food items in large buckets and trays were stored in walk in cooler uncovered and not protected from contamination. Cabbages were moldy and stored in the same cooler. Fans were on.
      • 2. Lids covering dry goods (beans, lentils, flour) are not the same ones that fit the containers, there is no seal.
      • Corrective Action(s): 1. All items in cooler need to be protected from contamination.
      • Cover buckets with lids available.
      • Use food grade wrap for large trays and buffet trays.

      • 2. Store all dry goods in pest proof containers with tight fitting lids.
