      • Corrective Action(s): All equipment must be kept clean when not in use. Clean and sanitize the equipment after each batch and cover the equipment to protect them from contamination.

      • Observation (CORRECTED DURING INSPECTION): Sliced pizza displayed inside warmer unit measured at 49.8C on the surface. Pizza slices were placed more than 2 hours ago. No time tracking system in place
      • Corrective Action(s): Discard the pizza slices. All cooked pizza slices must be held at a warmer >60C, or documentation showing time tracking of when pizzas were placed outside of 60C (2 hour time frame). Operator discarded the pizza. Operator intends to have documentation of time stamp

      • Observation: Rodent droppings observed to be in the corner of the dough making preparation table and behind the chest freezer at the back.
      • Corrective Action(s): Clean and sanitize the area with rodent droppings. Recommend to store all open flour and bags of flour inside containers to limit potential food sources. Connect with a professional pest control company if presence of rodents continue to be a problem.

      • Observation (CORRECTED DURING INSPECTION): Both hand wash stations (front and back) do not have paper towel.
      • Corrective Action(s): Operator refilled the paper towel dispenser. Ensure all hand wash stations are stocked with hot/cold water, soap and paper towels at all times.
