      • Observation (CORRECTED DURING INSPECTION): Large container of rice observed at room temperature. Rice was probed at 60C.
      • Corrective Action(s): Rice was transferred to shallow trays to cool down during the inspection.
      • *Cool hot foods from 60C-20C within 2 hours or less, and then from 20C to 4C or less to prevent pathogen growth. Cool rice in smaller portions and in shallow trays to speed up cooling.*

      • (3) Ice build up observed in upright white freezer beside cookline. Ice observed close to contacting food on top shelf.

      • Corrective Action(s): (1) Cover foods to protect from contamination.
      • (2) Discard single use containers. Remove bowls. Use scoops with a handle to protect food from contamination.

      • Observation: 1. Dish machine was not working at the time of inspection.
      • 2. No sanitizer was available and set-up during food preparation.
      • 3. One of the reach-in coolers was not functioning at the time of inspection - operator was aware and no food was stored in this unit.
      • Corrective Action(s): 1. Operator was instructed to set-up the 2-compartment sink for manual warewashing. Based on past compliance and dine-in situation at the facility (limited due to ongoing construction immediately outside restaurant), operator may use the two-compartment sink for manual washing until the dish machine is serviced.
      • 2. Operator mixed a sanitizer solution after this was brought up. 200 PPM chlorine residual was detected in the sanitizer container.
      • 3. Fix the cooler so at it is able to maintain at 4.0C or below.

      • Corrective Action(s): Note: These are repeated violation. If this violation is repeated again in the future, escalated enforcement actions will ensue.
      • 1. Move the eggs to a lower shelf or away from cooked food. Operator must ensure that food is stored in the proper hierarchy with raw food and raw proteins stored in a lower shelf or away from cooked food, ready-to-eat food or produce.
      • 2. Cover all food in the walk-in cooler. All food that is not being used for active service must be stored protected or with a cover to prevent potential cross-contamination.

      • Observation: 1. Food stored with no containers or protection directly in the side shelving units of the upright freezer.
      • 2. Most food in the reach-in coolers and walk-in cooler were stored uncovered/ unprotected.
      • 3. One bulk food containers was using an inappropriate dispenser (bowl) for dispensing food product.
      • Corrective Action(s): 1 and 2. All food must be protected by storing in a food-grade container or ziploc bag, etc. Food must NEVER be stored directly within a refrigeration or freezer unit unprotected to avoid potential contamination especially if the units are not cleaned. Not only must food be stored protected in an appropriate food-grade receptacle, they must be covered with either plastic wrap or lid to avoid overead contamination of condensate, dust, or any other potential contaminants.
      • Date to be corrected: Immediately - Next time any food unprotected from contamination will be discarded.
      • 3. Discard the single-use bowl and replace with a proper dispenser with a handle. Handle must never come in contact with the food item.

      • 2. Ice buildup noted in the upright freezer.
      • Corrective Action(s): Clean and sanitize the abovementioned areas and maintain in a sanitary condition at all time.
      • Remove all frozen food in the upright freezer and transfer to another functioning freezer. Defrost the ice buildup from the upright freezer and clean before returning frozen food into this unit.

      • Observation (CORRECTED DURING INSPECTION): < 10 ppm chlorine residual was detected in the chlorine sanitizer solution holding in-use wiping cloths.

      • Staff remixed the sanitizer solution once this issue was brought to their attention. 200 ppm chlorine residual was detected in the new sanitizer solution.
      • Change out the sanitizer solution every 2 hours, if not more often, to ensure it still contains the require concentration of chlorine residual (100 - 200 ppm) to allow food contact surfaces to be effectively sanitized during production.

      • 2. At least 2 frozen raw chicken stored with no packaging, unprotected, in the chest freezer.
      • 3. Improper dispenser (bowl), with no handle, was being used and stored within a bulk flour container.
      • Corrective Action(s): 1. Cover all food not in active use with either a plastic wrap, lid/ cover to prevent potential cross contamination of food.
      • 2. Raw chicken must be stored in appropriate packaging to avoid potential contamination of the chicken itself and other products that are stored along with the chicken as bacteria may be present on the surface of the chicken that could be transferred to other products. **Issue was corrected during the inspection** Raw chicken were placed in a bag for storage.

      • 2. Food (including raw fish, bags of lettuce and carrots) stored directly on the floor in the walk-in cooler.
      • Corrective Action(s): 1. Wrap or place a lid over all uncovered food that are in the reach-in coolers and walk-in cooler unit to protect them from potential contamination.
      • 2. Store all food in the walk-in cooler at least 6 inches off the floor. Food in a food service establishment must be stored at least 6 inches off the floor to facilitate cleaning and to allow for the monitoring of pests.

      • Observation: At least 4 ants were observed on the floor in between the kitchen handwash sink and dry storage rack for clean utensils.
      • As per operator, they use to have an external pest control company that would come in for pest issues. However, they were unaware of any pest issues and have since stopped the pest control company's regular inspection.
      • Corrective Action(s): Operator will attempt to address pest issue (2 weeks time given to contain issue). If operator cannot control the ant situation in his premises, then he is to contact the external pest control company to inform them of the ant issue noted in the kitchen. Work alongside the pest control company to address the pest issue.

      • 4. Dried on food soil noted on the shield of the blade of the handheld blender.
