      • Observation: Rodent droppings observed on floors along the perimeter of the kitchen and on various surfaces such as shelving and on top of the lids of food bins. Gaps observed at various locations of the ceiling, especially above the dishwashing area.
      • Corrective Action(s): Clean up all droppings and sanitize affected surfaces more frequently. Seal gaps and holes to prevent rodent entry. Sanitize food prep surfaces before starting food prep each day. Consult pest control company as needed. Correct as soon as possible.

      • Corrective Action(s): 1. Clean up droppings and sanitize affected surfaces. Seal gaps and holes to prevent rodent entry. Monitor rodent activity and contact pest control if signs of activity return. Correct as soon as possible.

      • Observation: Buns are being stored open and under the prep counter (used for meat and fish prep area) and next to garbage and compost bins.
      • Some fruit flies in this area.
      • Corrective Action(s): Ensure all food is being stored covered, away from garbage and not directly below food prep areas.

      • Observation: 1. Spice rack area: abundance of rodent droppings on the white cutting board shelf, on top of plastic wrapped sugar and spices and behind various plastic wrapped containers of food ingredients. Some droppings found inside sugar or spice container.
      • All tongs and utensils are stored directly below this shelf that is heavily contaminated.
      • 2. Dry storage room: some isolated droppings at the back of the wood chips and in back corners.
      • Corrective Action(s): Discard immediately all spices not in pest proof containers.
      • Remove all spices from this shelf and thoroughly clean and sanitize this area.
      • Clean and sanitize all equipment below.
      • Call a pest control contractor.
      • Ensure only pest proof containers are used.

      • Observation (CORRECTED DURING INSPECTION): Several cutting boards noted with deep cuts and impossible to clean and sanitize properly. Kitchen staff was chopping veggies on one cutting board with black marks and deep cuts. Operator switched to a new cutting board when asked.
      • Corrective Action(s): Remove cutting boards that have deep cuts, black marks and not in good working condition from the kitchen. Do not utilize this boards to prepare food as it is impossible to sanitize these borads.

      • Observation: 1- Trays of frozen cooked food observed siting on the floor inside walk in freezer. Food noted covered but stacked. This is not a food safe practice as all food should be off the floor by at least 6 inches.
      • Corrective Action(s): Protect food from potential contamination. Lift food trays off the floor at least 6 inch and organize the freezer in such a way that not overloaded.

      • Observation: Drain flies noted in dishwashing room. Operator stated that they follow cleaning and sanitation plan including use of biocidal liquids, flush drains etc. However, the issue persists.
      • Corrective Action(s): Ensure to perform a deep cleaning under dish pit and behind equipment. Remove grease and build ups behind dishwasher conveyer. Try to plug drains when it is not in use. Consider using bleach-water solution if compatible with your operation.
      • Contact your pest control company if the issue continues.

      • Observation:
      • 1- Range hood noted with accomulation of grease and dust. This may contaminate food and have a fire hazard.
      • 2- Several ceiling tiles noted out of place, damaged or not in proper place. Ceiling tiles should be in place to protect food and clean dishware from contamination from above.
      • Corrective Action(s): 1- Clean and maintain range hood. Remove dust and grease from the stainless steel grills.
      • 2- Repair or replace damaged, peeled or misplaced ceiling tiles in kitchen.

      • Observation: 1- Comperessor fan in walk-in cooler was leaking water. There were 2 containers to cattch water leak but this may introduce contamination to food stored below and at the side.
      • 2- Ice maker door falling apart. Screws are loose and may fall into the ice. This is a physical hazard to food. it is also potential spot for mold behind the door metal sheet wihich noted to be half taped. This is a repeated violation and may result in further enforcement actions if not corrected.
      • Corrective Action(s): 1- Remove any ready to eat food from below and close to water leak. Protect food from contamination.
      • Repair the leak and maintain.

      • 2- Repair ice maker door and maintain. Remove scotch tapes/packing tapes around the premeter of the door and seal properly. Temporary tape is not easily cleanable.

      • Racks that hold dishes in main kitchen have a build up of dust and debris.

      • Unlabelled chemicals found in bar area that staff were not able to identify.
      • Corrective Action(s): Do not use cardboard in fridges as it is dirty and not washable.
      • Thoroughly clean all racks and walk in coolers and ensure that raw meats are stored on shelves below ready to eat foods in walk in cooler.
      • Remove dirty styrofoam placed on corners of little chest freezer as it ios not cleanable.
      • Ensure that bar staff know what no rinse sanitizer they should be using in the bar area and that it is the proper concentration in a labelled bottle.

      • Corrective Action(s): Thoroughly wash and maintain all areas noted. Any racks that are peeling or severly rusted are to be refinished or replaced.
