      • Observation: Ice machine obeserved to have biofilm build up in the upper compartment. These conditions may promote microbial growth and cross contaminate ice.
      • Corrective action: Discard all ice. Wash/rinse/sanitize the interior of ice machine. (QUAT @200ppm) and let air dry. Ensure routine deep cleaning of unit.

      • Observation (CORRECTED DURING INSPECTION): 0ppm iodine observed in final rinse cycle of glass dishwasher at bar.
      • 12.5-25ppm iodine is required to ensure adequate sanitation.
      • Corrective action: sanitizer bottle was full but adjusted to allow adequate flow through pipe.
      • Ensure iodine test strips are used for daily monitoring of glass washer with a min 12.5-25ppm iodine.

      • Observation: 1) Mouse droppings were observed under sink at bar and under dry storage racks in back of kitchen.
      • 2) Ants observed under 1 compartment sink/microwave area in kitchen.
      • *Pests can transmit disease and efforts must be made to eliminate them.
      • Corrective action: Clean/sanitize all above noted areas. Connect with pest control company in implementing control measures and follow any recommendations.

      • Observation: Food debris and dust build up in hard to reach areas including under reach in coolers, inside walk in freezers, under dry storage, etc. These conditions may attract pests and also cross contaminate food.

      • Observation:
      • Found cracked and chipped plastic food lids and containers. Bacteria will live in the cracks and pieces of plastic could fall into food.
      • Corrective Action(s):
      • Throw away any damaged lids, containers, and/or equipment and purchase new ones.

      • Observation (CORRECTED DURING INSPECTION): Paper towels werer not present at the handsink inside the bar at the time of inspection.

      • Corrective Action: Have single use towels available in the handsink at all times. Do not use cloths to dry hands because cloths can harbour bacteria when they are moist. In the meantime, staff agreed to use disposable napkins when drying hands.

      • Observation: 1) Lids used for covering inserts had oil residue on them

      • Corrective Action: Remove the ice, clean and sanitize the ice machine to ensure that the ice is free from contamination.
      • 3) Food stains and debris were observed on the floors inside the walk-in cooler and in the dry storage area

      • Observation: - There is accumulated dust on the ceiling tiles; around air diffusers; and along walls + surfaces 2ft. below the ceiling in the kitchen. This dust could fall into food. Wash these areas to remove the dust, and ensure this is done on a regular basis. Date To Be Corrected By: November 8, 2018

      • Observation: - The microwave door is broken on the bottom left corner. There is burn marks + melting plastic on the inner door area. This is unsafe and a fire hazard. Discard the microwave and purchase a commercial grade microwave. Date To Be Corrected By: November 8, 2018.

      • Observation:
      • There is an unsealed, wooden shelf in the kitchen by the 1-compartment, food prep. sink. Unsealed, wood is not cleanable and will absorb food. This will attract pests.

      • Paint the shelf or permanently install a shelf of proper material (durable, food grade, cleanable, sealed, and level).

      • Observation:
      • The walls, by the new upright coolers, have chipped paints and damaged drywall. The wall is not sealed or cleanable.

      • Repair and paint the wall so that it is durable, smooth, level, cleanable, and sealed.

      • Observation:
      • 1) There is some accumulated grease and food debris along the back wall behind the cooking equipment (fryers, grill, stove). This will attract pests.
      • 2) The interior walls and surfaces of the ice making machine has mold and mildew on it. This can fall into the ice and contaminate it.
      • Corrective Action(s):
      • 1) Move cooking equipment away from walls and clean the floor beneath and behind the equipment to remove the grease and food. To encourage cleaning this area, install wheels on equipment to make it easier to move away from the walls. Also, ensure to install flexible gas hoses.
      • 2) Empty the ice machine and clean all walls and working parts of the ice machine on a more regular basis with soap and water, and sanitize with bleach and water.

      • A disconnected waste pipe in the dry storage room is blocked with a rag. Sewer gases are exiting out of the pipe into the room.

      • Properly cover or "cap off" the waste pipe to prevent sewer gases from exiting and accumulating in the room.
