      • - Steps are not being taken to protect the contamination of the bulk supply of scrub gel. Please ensure that measures are put in place so that scrub gel is dispensed without contaminating the bulk supply.-None of the bottles in the pedicure service area were labelled.Please ensure all containers and bottles are labeled.

      • - The sink in the service area was decontaminated to be used for reprocessing reusable tools.The sign illustrating "Do not wash hands in the sink" was still not posted

      • Written procedure for client care in case of accidental cut and puncture was not available.
    • 06. Do operators and personal services workers comply with the Personal Services Regulation and Standards and do they have the skills and knowledge to perform their duties in a safe and sanitary manner?
      • -Personal Services workers lacked the skills and knowledge to perform their duties in a safe and sanitary manner; they were not able to classify equipment as non-critical, semi-critical and critical; they were not following manufacturer’s instructions; they lacked the understanding of infection/injury risk associated with the personal service being provided.
    • 09. Is a point of service risk assessment being performed of the condition of the client and of the equipment to be used on the client prior to performing a personal service?
      • -There was no point of service risk assessment being done of the client and equipment prior to performing a personal service. Please ensure that the condition of a client’s skin, hair, nails, teeth or body as applicable is assessed for signs of infection, infestation or irritation prior to performing a personal services.Please ensure equipment is assessed to ensure it is visibly clean and in good condition and repair.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • - Single-use equipment (Nail filers) was not being discarded after each use. Equipment must be discarded after use on one client if the manufacturer labels it as: single-use, not for reuse, do not reuse, disposable, discard after single-use, or do not use twice.- Porous or uncleanable equipment (wooden foot paddle) was not being discarded after each use. Equipment classified as porous or uncleanable must be discarded after use in one personal service on one client.
    • 15. Is reusable equipment handled and stored properly (Critical)?
      • - Equipment is not being protected from contamination during handling and storage. Ensure equipment is stored in a manner to keep it clean and free from contamination.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • - Used semi-critical equipment were not being adequately cleaned and dried prior to disinfection or sterilization. Ensure tools are: disassembled, cleaned to remove soil, and rinsed and dried prior to disinfection or sterilization.
    • 20. Is reusable equipment disinfected properly?
      • - Semi-critical equipment is not being adequately cleaned and disinfected. Ensure semi-critical must be cleaned and disinfected using, at a minimum, a high-level disinfectant.- High level disinfectants are not being tested and verified daily using proper test strips. Ensure the high level disinfectant is tested daily using the test strips specified by the disinfectant manufacturer.- Disinfection records were not available. Ensure records are kept of: test dates, whether the solution passed/failed, when the disinfectant was changed and the worker’s initials who performed the activity.

      • - Work surface areas for contaminated equipment and/or cleaned equipment are not adequate. Ensure that adequate counter space is available for: the storage of unclean equipment and drying/disinfection of clean equipment.
    • 31. Are sinks accessible and used appropriately for the personal services provided?
      • - Handwashing was occurring at a sink used for equipment reprocessing. The operator also mentioned that the wastewater after the pedicure services was dumped into the sink. The operator was advised to use the washroom sink for handwashing and use the only sink available in the service area for reprocessing semi-critical tools. The operator was also advised to put a sign illustrating "Do not wash hands in the sink".
