- 03. Are clients at this facility being protected from hazards (Critical)?
- The following critical hazards were observed:- Knives were stored unsecured in a kitchen drawer accessible to children. Please ensure knives are stored secured or out of children’s reach.-There were sharp screws exposed on the kitchen play set in the Toddler Room. Please address.-There was a storage shelf in the Kindergarten Room that was not secured to the wall. Please secure.-There were loose tacs and staples on the wall of the Pre-school Room. The use of push pins and staples are not recommended in child care facilities. Please consider removing all push pins and staples and switching to alternatives such as: tape, sticky tac, Velcro etc. If staples are to be used they should be covered with tape.- Small loose parts were observed in the Toddler Room; these are a choking hazard. Please remove.

- -The door to the kitchen was broken and left open, allowing children to access. Please repair the door and ensure children do not have access to the kitchen.

- -Diaper supplies were not being stored separate for each child in the Baby or Toddler Room’s. Please ensure supplies are stored separate.

- The following concerns were noted with diapering at this facility:- The diaper table in the Toddler Room was being used for storage. This is a contaminated space; do not use for storage.-The diaper mat in the Baby Room was torn. Please replace.
- 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
- -The diaper table in the Toddler Room was not installed next to a designated sink. Please ensure the table is next to a handsink to promote handwashing. The sink needs to be used for handwashing only.- Staff were rinsing dishes at the diapering sink. This is a contaminated space and needs to be used for diapering only.
- 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
- -Chlorine was at 50ppm (too low) in the washroom and Baby Room and 500ppm (too high) in other rooms. Please ensure the same concentration of Chlorine solution is used throughout. This should be verified with test strips. *Ensure Chlorine bottles are labelled with the appropriate concentration

- -The light table in the OSC Room was cracked/broken. Please remove/replace.
- 21. Is the plumbing system adequate and being properly maintained (Critical)?
- -One of the washroom sinks at the main entrance was not working. Please repair.
- 30. Are workers washing hands when appropriate and/or in a proper manner?
- -The hand sanitizer in the Toddler Room expired in 2023. Please remove/replace.

- -The diaper mat in the Pre-school washroom had no safety belt and was not secured to the counter. Please install a belt and ensure the table is secured to the counter so it doesn’t slide.

- -The Chlorine sanitizer in the Toddler Room or the Pre-school washroom was not labelled. Please label.

- 04. Are clients at this facility being protected from hazards (Non-Critical)?
- There were chemicals such as Mr. Clean disinfectant and Lysol sprays stored in unlocked cabinets under the sinks in the OSC and Preschool washrooms. Please put a lock on these cabinets.
- 06. Are proper protocols being followed in regards to client care?
- The facility does not have chlorine or QUAT test strips. Please purchase these test strips and ensure all disinfectants are tested when they are prepared. Chlorine (bleach) solutions must be at 100-200 ppm chlorine and QUAT disinfectant must be at a minimum of 200 ppm.

- The bleach water spray bottle in the toddler room was measured at 25 ppm chlorine. Upon request, this was remade and measured at 100 ppm. Please ensure all bleach sanitizers are at 100-200 ppm chlorine.
