- Observation (CORRECTED DURING INSPECTION): Less than 200 ppm QUATS sanitizer was measured inside both sanitizer pails, one of the two sanitizer spray bottles, and from the dispenser. A hole was present in the hose connected to the QUATS sanitizer dispenser.
- Corrective Action(s): Ensure all food contact surfaces are washed, rinsed, sanitized in 200 ppm QUATS sanitizer for at least 1 minute, and then air dried. Manufacturer's label directions for the QUATS sanitizer used on-site specify a 200 ppm active concentration and a minimal 1 minute contact time.

- Staff manually diluted the QUATS sanitizer to setup 200 ppm in the sanitizer pails to sanitize surfaces and informed they would change the sanitizer every 2 hours. 200 ppm QUATS sanitizer was available from the dispenser upon adjusting the water temperature setting by the Manager who arrived later during the inspection.

- Observation (CORRECTED DURING INSPECTION): 1. Meat patties were observed uncovered and placed directly inside the chest freezer that had some ice buildup present inside of it.
- 2. Ice cream was observed uncovered inside the walk-in-freezer in an area that had ice buildup present on the ceiling above it.
- Corrective Action(s): 1. Shift supervisor placed the patties inside a food container and covered it with saran wrap during the inspection. Ensure food is kept covered in storage areas.

- Observation: 1. Light bulb inside the walk-in-freezer was not encased in a shatterproof light cover.
- 2. Liquid soap dispenser for the handsink at the front service area is currently mounted to the inside of the cabinet below the handsink which is not a convenient location to access it for handwashing purposes.
- 3. Broken floor tiles have not been replaced yet. Shift Supervisor stated that they are planning to replace the floor tiles with the upcoming renovation with a time-frame of 6 months.
- Corrective Action(s): 1. Ensure the light bulb inside the walk-in-freezer is placed inside a shatterproof light cover; Correct as soon as possible within 3 months.
- 2. Ensure the liquid soap dispenser is re-located/wall-mounted near the handsink in the front service area where it is easier to access for handwashing purposes (i.e. without opening the cabinet drawer each time to apply liquid soap); Correct by March 12, 2022.
- 3. Replace all broken tiles with intact tiles; Correct within 6 months. If renovations are proposed (e.g. to equipment, handsinks, ware-washing, food service area, or layout etc.), three hardcopies of the floor plan outlining the renovations proposed are required for health approval prior to making any changes to the existing floor plan.
