      • Observation: Numerous rat droppings found throughout facility; kitchen and storage room. Operator is fully aware of the situation and had pest control company come in but they only set up small mouse traps. Pest control company is for the building and visits may be once a year.
      • Corrective Action(s): Required operator to remove all droppings in the premises; ensure to wet them with sanitzer prior to removing them. Once removed, clean and sanitize the premises thoroughly. Ensure to perform regular monitoring and cleaning. Required operator to discuss pest management visits with the landlord to increase the visits or hire own pest control for the restaurant only.

      • 3. Ingredients in original packaging were placed on the floor of the walk-in cooler after shelving was cleaned.
      • Corrective Action(s): 1. Chicken liver was moved to the bottom shelf of the domestic cooler.
      • 2. Soup stock was covered with a food-grade lid.
      • 3. Ingredients were placed back on the shelving of the walk-in cooler.

      • 2. High temperature dishwasher not sanitizing dishes with adequate temperature. After multiple attempts, the temperature only reached maximum 43 degrees Celsius at final rinse.
      • Corrective Action(s):
      • 1. Prepare a 100-200 ppm bleach sanitizer (half teaspoon bleach per litre water) to sanitize all food contact surfaces.
      • 2. Use the manual warewashing method to wash, rinse and sanitize (200 ppm bleach) dishes.
